LOCAL CONTROL WEST (LCW) CLEARED AN EMBRAER E145 FOR TAKEOFF ON RUNWAY 30 FULL LENGTH. AN EMBRAER E145 WAS CLEARED TO LAND RUNWAY 1C. THE RUNWAY 30 E145 APPEARED TO DELAY TAKEOFF ROLL AND WHEN HE STARTED HIS ROLL THE LANDING E145 CROSSED RUNWAY 30 OVER THE DEPARTING E145. CLOSEST PROXIMITY REPORTED BY ATCT WAS 300 FEET VERTICAL. ASDE-X ANALYSIS REPORTED THE ALTITUDE AS APPROXIMATELY 100 FEET AGL. (ASDE-X ALTITUDE ERROR IS REPORTED TO BE PLUS OR MINUS 100 FEET.) THE COMPUTED 3-DEGREE GLIDESLOPE ALTITUDE IS 134 FEET. E145 TAIL HEIGHT IS 22 FEET 2 INCHES.

LOCAL CONTROL (LC) INSTRUCTED A CESSNA C172 TO TURN RIGHT ON TAXIWAY A4 AFTER LANDING RUNWAY 6. IT APPEARED THAT THE C172 WAS TURNING AND A CANADAIR CRJ7 WAS INSTRUCTED TO TAXI INTO POSITION AND HOLD (TIPH) ON RUNWAY 6 FOR FULL LENGTH DEPARTURE. LC THEN ISSUED TRAFFIC TO AN AIRCRAFT IN THE PATTERN. LC SCANNED THE RUNWAY AND THOUGHT IT WAS CLEAR THEN ISSUED TAKEOFF CLEARANCE TO THE CRJ7. SHORTLY AFTER THE CRJ7 BEGAN TAKEOFF ROLL THE C172 PILOT STATED HE MISSED A4 AND WOULD LIKE TO TURN ON BRAVO. THE CRJ7 PILOT HEARD THIS TRANSMISSION, LOOKED OUT AND SAW THE C172 ON THE RUNWAY, AND ABORTED TAKEOFF USING THRUST REVERSERS TO SLOW THE AIRCRAFT AND VEERING TO THE LEFT TO MISS THE C172. CLOSEST PROXIMITY AS REPORTED BY THE CRJ7 WAS 10 FEET WING TIP TO WING TIP. BASED ON PRELIMINARY REVIEWS OF VOICE DATA, IT APPEARS THE CRJ7 ENCOUNTERED THE C172 AT A POINT BEFORE THE INTERSECTION WITH RUNWAY 31, APPROXIMATELY 2500 FEET FROM RUNWAY 6 APPROACH END.

A PIPER PA46 LANDED RUNWAY 29R AND WAS INSTRUCTED TO TURN OFF AT TAXIWAY B3. THE PA46 PILOT REPORTED HE COULD NOT MAKE THAT TAXIWAY AND WAS INSTRUCTED TO TURN OFF AT B5. B5 IS NOT LIT BUT HAS REFLECTORS AND IS 4,417 FEET FROM THE APPROACH END. LOCAL CONTROL (LC) THEN CLEARED A CANADAIR CRJ2 TO LAND ON RUNWAY 29R. THE CRJ2 PILOT ADVISED THE TOWER ON LANDING ROLL THAT SOMEONE WAS STILL ON THE RUNWAY. LC ASKED THE PA46 TO VERIFY HE WAS ON B5 AND THE PILOT SAID HE WAS HAVING TROUBLE FINDING THE TAXIWAY. THE CRJ2 PILOT LATER STATED THAT THEY OBSERVED THE PA46 AFTER TOUCHDOWN DURING THE DEPLOYMENT OF THE REVERSERS. THE CRJ2 USED MAX BREAKING AND AS THE PA46 WAS WELL TO THE LEFT SIDE OF THE RUNWAY, THEY VEERED TO THE RIGHT TO AVOID CONTACT WITH THE WINGS AND ESTIMATE THE TWO AIRCRAFT MISSED BY 15 FEET WING TIP TO WING TIP. CLOSEST PROXIMITY REPORTED AS 15 FEET. APPROXIMATELY POSITION OF THE PA46 WAS NEAR B4.

LOCAL CONTROL (LC) AND GROUND CONTROL (GC) WERE COMBINED. LC CLEARED A CESSNA C550 TO LAND ON RUNWAY 31. TWO MINUTES LATER A TRACTOR VEHICLE REQUESTED TO PROCEED FROM THE TERMINAL RAMP TO THE NORTH RAMP. LC ISSUED INSTRUCTIONS TO CROSS RUNWAY 31 AT TAXIWAY DELTA. TWO MINUTES LATER THE C550 PILOT REPORTED TO LC THAT THEY HAD COLLIDED WITH THE TRACTOR AT THE INTERSECTION OF RUNWAY 31 AND DELTA. THE LEFT WING HIT THE TRACTOR AS THE VEHICLE DROVE NORTHEAST BOUND ON THE RUNWAY FROM DELTA. THERE WERE NO INJURIES OR FIRE.

A DEHAVILLAND DH8A WAS ISSUED PROGRESSIVE TAXI INSTRUCTIONS TO RUNWAY 24R VIA LIMA, ROMEO, TO HOLD SHORT OF RUNWAY 24L. THE DH8A PILOT READ BACK INSTRUCTIONS AND THE HOLD SHORT FOR RUNWAY 24L. THE DH8A SWITCHED TO LOCAL CONTROL (LC) AS INSTRUCTED. LC ATTEMPTED TO STOP THE DH8A BEFORE ENTERING RUNWAY 24L AT ROMEO DUE TO A CANADAIR CRJ2 ON SHORT FINAL RUNWAY 24L. THE CRJ2 WAS CROSSING LANDING THRESHOLD AND INITIATED A GO-AROUND ON HIS OWN AND FLEW OVER THE DH8A WITH THE CLOSEST PROXIMITY OF 200 FEET VERTICAL. AMASS ANALYSIS SHOWS THAT THE DH8A WAS APPROXIMATELY 104 FEET FROM RUNWAY CENTERLINE AS THE CRJ2 PAST ROMEO IN A CLIMB. RUNWAY WIDTH IS 150 FEET AND CRJ2 WINGSPAN OF 70 FEET.

LOCAL CONTROL 10 (LC10) CLEARED A EMBRAER E145 FOR TAKEOFF ON RUNWAY 32L AT INTERSECTION MIKE OVERLOOKING THE LEARJET LJ25 LANDING ON RUNWAY 9R. THE LOCAL MONITOR RECOGNIZED THE CONFLICT AND INSTRUCTED LC10 TO KEEP THE E145 LEVEL AND FOR THE NORTH LOCAL CONTROLLER TO ISSUE GO AROUND INSTRUCTIONS TO THE LJ25. THE LJ25 PASSED JUST BEHIND AND ABOVE THE E145 AT THE INTERSECTION OF RUNWAY 9R ARRIVAL PATH AND RUNWAY 32L APPROXIMATELY 430 FEET AGL. THE E145 WAS PASSING THROUGH THE SAME AREA AT APPROXIMATELY 90 FEET AGL. THE CLOSEST PROXIMITY IS REPORTED AS 340 FEET VERTICAL AND 150 FEET LATERAL.

RUNWAY 1/19 HAD BEEN CLOSED BY OPS. LOCAL CONTROL (LC) PROPERLY PLACED THE 2 RUNWAY INCURSION DEVICES (RID) IN THE LC POSITION BUT DID NOT INFORM THE TRACON OF THE CLOSURE. TRACON CALLED 27 MINUTES LATER REQUESTING A VISUAL APPROACH TO RUNWAY 19 FOR A LEARJET LJ45. LC APPROVED THE REQUEST. THE LJ45 WAS CLEARED TO LAND RUNWAY 19, OVER FLYING 2 EMPLOYEES LOCATED NORTH OF THE DISPLACED THRESHOLD, ON RUNWAY 19, BETWEEN TAXIWAYS L/A AND B. THE DISPLACED THRESHOLD IS 770 FEET FROM THE APPROACH END OF RUNWAY 19. THE LJ45 FLEW OVER THE PEDESTRIANS AT AN ESTIMATED 150 FEET VERTICAL AND TOUCHED DOWN SLIGHTLY PAST TAXIWAY QUEBEC, 1800 FEET FROM 19 THRESHOLD. THE LJ45 EXITED THE RUNWAY AT JULIET. DISPLACED THRESHOLD IS 771 FEET FROM THE APPROACH END OF RUNWAY 19. TWO (2) VEHICLES WERE ON THE RUNWAY ABEAM TAXIWAY B, APPROXIMATELY 400 FEET FROM APPROACH END. TYPICALLY THE RAISED "X" IS ON THE TRUCK. THE RADIOS TUNED TO LC ARE IN THE TRUCKS. AIRPORT PERSONNEL DID NOT HAVE HAND HELD RADIOS OUT ON THE RUNWAY AND BOTH TRUCKS HAD THEIR BEACONS (AMBER) ON. TRUCK CARRYING THE "X" WAS POSITIONED ABEAM TAXIWAY B IN THE MIDDLE OF THE RUNWAY AND THE "X" WAS BEING RAISED. SUPERVISOR WAS FACING THE EMPLOYEE RAISING THE "X" AND SAW THE LJ APPROACHING. SUPERVISOR PROCEEDED TO HIS TRUCK WHICH WAS CLOSER TO THE EDGE BUT STILL ON THE RUNWAY ABEAM B AND AS HE WAS TRANSMITTING TO TOWER REGARDING THE LJ, THE AIRCRAFT WAS DESCENDING OVER HIS POSITION. HE CLEARLY SAW THE DETAIL OF THE UNDERSIDE OF THE AIRCRAFT AND ESTIMATES THE OVER FLY WAS 100 FEET. IN ADDITION THERE WAS A RAISED "X" (NO VEHICLE INVOLVED) 850 FEET FROM DEPARTURE END (ON THE NUMBERS) WHICH WAS FULL OF FUEL.

GROUND CONTROL (GC) REQUESTED FROM LOCAL CONTROL (LC) TO CROSS RUNWAY 35R AT TAXIWAY GOLF WITH A PIPER PA32. LC ADVISED GC TO CROSS AFTER A CESSNA C414 AIRCRAFT THAT WILL GO INTO POSITION ON 35R ABEAM TAXIWAY MIKE AND DEPART. WHEN THE C414 PASSED GOLF, GC INSTRUCTED THE PA32 TO CROSS 35R AT GOLF. A CIRRUS SR22 WAS ON SHORT FINAL 35R AT THE TIME WHEN ATCT SUPERVISOR ADVISED BOTH LC AND GC OF THE SR22. GC ATTEMPTED TO STOP THE PA32 BUT THE AIRCRAFT HAD ALREADY CROSSED THE RUNWAY EDGE. LC WAS INSTRUCTED TO ISSUE A GO AROUND TO THE SR22. AT THAT TIME THE SR22 HAD CROSSED LANDING THRESHOLD AND INITIATED A GO AROUND ON HIS OWN AND LC ISSUED GO AROUND INSTRUCTIONS. CLOSEST VERTICAL PROXIMITY WAS AN ESTIMATED 100 FEET AS THE PA32 WAS CROSSING THE RUNWAY AT GOLF (APPROXIMATELY 2,800 FEET FROM APPROACH END 35R) AND THE SR22 WAS PASSING OVERHEAD. ATCT COULD NOT DETERMINE IF A DIRECT OVERFLIGHT OCCURRED.
