A319 WAS CLEARED TO LAND ON RUNWAY 12R. DURING LANDING ROLL, ASDE-X ALERTED FOR AN UNKNOWN TARGET ON RUNWAY 12R. LOCAL CONTROL (LC) ISSUED A WARNING TO THE A319, AND ADVISED THE UNKNOWN TARGET COULD BE FALSE. (DUE TO SNOW AND REDUCED RVR, THE EVENT WAS NOT VISIBLE FROM THE TOWER). A319 ADVISED LC THAT THE TARGET WAS REAL, AND REPORTED SEEING A PICKUP TRUCK ON THE RUNWAY APPROXIMATELY 500-600 FT AHEAD OF HIM. A319 WAS BY THEN AT TAXI SPEED, AND STOPPED ON RUNWAY 12R, UNTIL THE PICKUP TRUCK EXITED THE RUNWAY. A319 EXITED RUNWAY 12R AT TAXIWAY ALFA 5. CLOSEST PROXIMITY IS WAS AT 0 FT VERTICAL AND 120 FT LATERAL, PER ASDE-X ANALYSIS.

HXB WAS CLEARED FOR THE OPTION AND WAS DEPARTING/TOUCH AND GO ON RUNWAY 9. PA28 WAS CLEARED FOR THE OPTION FOR RUNWAY 33. HXB PASSED THROUGH THE INTERSECTION OF THE TWO RUNWAYS APPROXIMATELY 50 FEET ABOVE THE RUNWAY. PA28 PASSED APPROXIMATELY 200 FEET OVER THE HXB WITH 0 FEET LATERAL SEPARATION. THE DISTANCE WAS ESTIMATED BY THE FRONT LINE MANAGER, WHO HAD JUST REPORTED FOR WATCH SUPERVISION DUTY IN THE TOWER. LOCAL CONTROL (LC) DID NOT ISSUE TRAFFIC TO EITHER AIRCRAFT. NEITHER PILOT COMMENTED ON THE PROXIMITY OR APPEARED TO TAKE EVASIVE ACTION. DURING A SUBSEQUENT INTERVIEW WITH THE AIR TRAFFIC MANAGER, LC DID NOT APPEAR TO RECOGNIZE THE OPERATION DID NOT MEET CROSSING RUNWAY SEPARATION REQUIREMENTS.

B350 WAS ISSUED TAXI INSTRUCTIONS FOR A RUNWAY 36C DEPARTURE AND WAS INSTRUCTED TO HOLD SHORT OF RUNWAY 36R AT TAXIWAY ROMEO. B734 WAS LANDING RUNWAY 36R. AS THE B734 PASSED TAXIWAY DELTA 3, GROUND CONTROL (GC) INTENDED TO CLEAR THE JS31 ACROSS RUNWAY 36R BEHIND THE LANDING B734 BUT MISTAKENLY ISSUED A CLEARANCE FOR THE B350 TO CROSS RUNWAY 36R WITHOUT DELAY. GC REALIZED THE MISTAKE AND IMMEDIATELY INSTRUCTED THE B350 TO HOLD SHORT OF THE RUNWAY. B350 WAS ACROSS THE HOLD SHORT LINE AS B734 PASSED TAXIWAY ROMEO. B734 CALLED THE TOWER AND STATED THAT THE B350, “ALMOST HIT US.” THE ASDE-X DID NOT ALARM.

FIRST CRJ7 CALLED GROUND CONTROL (GC) AT TAXIWAY GOLF, HOLDING SHORT OF TAXIWAY TANGO. GC ISSUED INSTRUCTIONS TO CRJ7 TO TAXI VIA TAXIWAYS TANGO, ALPHA 7 AND BRAVO. CRJ7 WAS ON TAXIWAY TANGO AND CROSSING THE EXTENDED CENTERLINE FOR RUNWAY 9R PROTECTED AREA APPROXIMATELY 1327 FEET FROM THE RUNWAY THRESHOLD WHILE A SECOND CRJ7 WAS LANDING . THIS AREA IS PROTECTED FOR ALL AIRCRAFT IN ALL WEATHER CONDITIONS DURING EASTBOUND OPERATIONS. TAXIING CRJ7 PASSED THE EXTENDED RUNWAY CENTERLINE JUST PRIOR TO THE ARRIVING CRJ7 REACHING THE TAXIWAY AND ASDE-X ANALYSIS INDICATES THE WING OF THE ARRIVING CRJ7 PASSED APPROXIMATELY 85 FEET ABOVE THE TAIL OF THE TAXIING CRJ7.

H500 (HELO) WAS CLEARED FOR THE OPTION TO RUNWAY 19. H500 TOUCHED DOWN ON RUNWAY 19 APPROACH END THEN LIFTED OFF AND HOVERED OVER THE APPROACH END OF RUNWAY 19, APPROXIMATELY 3 FEET ABOVE THE PAVEMENT. LC SAW THE H500 LIFT OFF, BUT DID NOT SEE THE H500 STOP IN A HOVER. T38 CALLED ON UHF FREQUENCY INBOUND FOR RUNWAY 19. LC CLEARED THE T38 FOR THE OPTION RUNWAY 19. T38 EXECUTED A LOW APPROACH ON RUNWAY 19, APPROXIMATELY 300 FT ABOVE THE STILL-HOVERING H500. TRAFFIC WAS NOT EXCHANGED. NEITHER AIRCRAFT REPORTED SEEING THE OTHER AIRCRAFT.

LOCAL CONTROL (LC) INSTRUCTED HXA TO LINE UP AND WAIT ON RUNWAY 31R. HXA READ BACK "TAKEOFF RUNWAY 31R NO DELAY". LC FAILED TO CATCH THE INCORRECT READ BACK. LC THEN INSTRUCTED C152 TO CROSS RUNWAY 31R AT TAXIWAY DELTA. CONTROLLER IN CHARGE (CIC) OBSERVED THE OPERATION AND TOLD LC "NEGATIVE, NEGATIVE.” HXA HAD ALREADY ROTATED AND COULD NOT BE STOPPED. HXA OVER FLEW C152 BY 100 TO 200 FEET. ATC TRAINING WAS IN PROGRESS.

PA32 WAS ISSUED A LAHSO LANDING CLEARANCE ON RUNWAY 24. LAHSO CLEARANCE WAS ACKNOWLEDGED WITH AN ACCEPTANCE AS THE "ACTIVE" LAHSO AIRCRAFT. ATC INFORMED C750 OF THE LAHSO. C750 WAS GIVEN A FULL-LENGTH LANDING CLEARANCE TO RUNWAY 33 (PASSIVE LAHSO AIRCRAFT). ATC NOTICED THAT THE PA32 WAS ACCELERATING RATHER THAN DECELERATING AFTER TOUCHING DOWN ON RUNWAY 24, AND SUSPECTED THAT THE PA32 WAS NOT GOING TO COMPLY WITH THE LAHSO CLEARANCE. UPON HEARING ATC'S URGENT CALLS TO THE PA32 TO HOLD SHORT OF RUNWAY 33, THE C750 ASSUMED THE "ACTIVE" LAHSO ROLE AND TOOK EVASIVE ACTION, STOPPING SHORT OF RUNWAY 24 AT THE LAHSO MARKING AS THE PA32 PASSED THROUGH THE INTERSECTION. C750 RIGHT SEAT PILOT WAS OBSERVING THE PA32 AND PROMPTED THE PILOT FLYING TO ANTICIPATE HARD BRAKING TO AVOID ENTERING THE INTERSECTION. C750 STOPPED AT THE LAHSO LINE ON RUNWAY 33. ESTIMATED CLOSEST PROXIMITY WAS 215 FEET. WHEN PA32 REACHED TAXIWAY HOTEL IT VEERED OFF THE RUNWAY INTO THE GRASS ON THE NORTH SIDE AND CAME TO A STOP AND BECAME STUCK.

GROUND CONTROL (GC) ISSUED TAXI CLEARANCE TO BE95 (ANGEL FLIGHT) FOR RUNWAY 15 INTERSECTION DEPARTURE FROM TAXIWAY HOTEL. READ BACK WAS CORRECT. AS C172 WAS TOUCHING DOWN ON RUNWAY 15 JUST PRIOR TO THE TAXIWAY HOTEL INTERSECTION, LOCAL CONTROL (LC) OBSERVED THE BE95 CROSS THE HOLD LINE AT TAXIWAY HOTEL. LC DID NOT ISSUE GO AROUND INSTRUCTIONS BECAUSE HE JUDGED IT WAS TOO LATE AND SAFER TO ALLOW THE C172 TO CONTINUE. GC INSTRUCTED THE BE95 TO STOP AND THE AIRCRAFT CAME TO A FULL STOP JUST SHORT OF THE RUNWAY EDGE LINE AS THE C172 WAS ROLLING PAST THE INTERSECTION. CLOSEST HORIZONTAL PROXIMITY WAS APPROXIMATELY 36 FEET FROM THE C172 WING TIP TO EDGE OF RUNWAY 15. THIS EVENT OCCURRED AT HOT SPOT 3.
