LC SUBSEQUENTLY INSTRUCTED THE P28A TO PULL UP AND HOLD SHORT OF RUNWAY 26R. THE PILOT READ BACK HOLD SHORT OF RUNWAY 26R BUT DID NOT USE HIS CALL SIGN. THE P28A SUBSEQUENTLY CROSSED THE RUNWAY 26R HOLD BARS AND TAXIED ONTO THE RUNWAY WITHOUT A CLEARANCE. LC INSTRUCTED TRAFFIC ON SHORT FINAL A/C 2/RV7 TO GO AROUND. THE RV7 FLEW OVER THE P28A ON THE GO AROUND, APPROXIMATELY 100 FEET ABOVE THE P28A. GO AROUND WAS ISSUED INSIDE A HALF MILE FINAL. NOT A HOT SPOT EVENT.

WHEN GROUND CONTROL WAS ALERTED TO THE CONFLICT, HE INSTRUCTED VEH#1 TO STOP, WHICH PUT THE VEHICLE IN THE MIDDLE OF THE APPROACH AREA FOR RUNWAY 28L. ATC REPORTED THAT A/C#1 FLEW DIRECTLY OVER VEH#1 ON APPROACH AND CLEARED VEH#1 BY ABOUT 10 FEET. IN A CONVERSATION WITH THE PILOT OF A/C#1, THE PILOT REPORTED THAT HE SAW THE VEHICLE, TRIED NOT TO FLY DIRECTLY OVER THE TOP, AND THAT HE WAS 50? AGL WHEN HE PASSED VEH#1. A/C#1 SUBSEQUENTLY LANDED ON RUNWAY 28L. BIL STANDARD OPERATING PROCEDURES PARAGRAPH 4-4-6 REQUIRES ATC TO PROTECT THE FAR PART 77 APPROACH SURFACE OVER TAXIWAY GOLF FOR AIRCRAFT AND VEHICLES 11?3? OR TALLER. VEH#1 REPORTED HEIGHT IS 11?4? TALL AND EXCEEDS THIS HEIGHT RESTRICTION. THE DISTANCE FROM THE APPROACH END OF RUNWAY 28L TO THE CENTERLINE OF TAXIWAY GOLF IS APPROXIMATELY 475?.

    GROUND CONTROL CLEARED VEH 1 ONTO THE FIRST 4000 FEET OF RUNWAY 02 TO SWEEP THE RUNWAY. GROUND CONTROL AND LOCAL CONTROL POSITIONS WERE COMBINED; HOWEVER, THE POSITIONS WERE WORKED SEPARATELY SO A VEHICLE ON GROUND CONTROL COULD NOT HEAR WHAT ATC TRANSMITTED ON LOCAL CONTROL. ALSO, AN AIRCRAFT ON LOCAL CONTROL COULD NOT HEAR WHAT ATC TRANSMITTED ON GROUND CONTROL. A/C 1 CHECKED ON LOCAL CONTROL FREQUENCY OUTSIDE OF WESIE AND WAS CLEARED TO LAND ON RUNWAY 20. VEH 1 WAS STILL ON THE LAST 4000 OF RUNWAY 20. A/C 1 LANDED ON RUNWAY 20 AND REPORTED THAT THEY WERE ABOUT 1500 FEET FROM THE END AND THERE WAS A TRUCK ON THE RUNWAY. ATC INSTRUCTED A/C 1 TO MAKE A 180 ON THE RUNWAY AND CLEAR AT ALPHA TO THE RAMP. DURING A PHONE CONVERSATION WITH A/C 1, A/C 1 REPORTED THAT THEY SAW THE VEHICLE LIGHTS HALF WAY THROUGH BRAKING AND THAT THE CLOSEST PROXIMITY WAS 200 FEET.

A/C 1 PROVIDED A GOOD READBACK. A/C 2/C421 INITIALLY REQUESTED TAXI FOLLOWING A PRECEDING LEAR JET. GC INSTRUCTED "FOLLOW THE LEAR JET THERE, BRAVO, CROSSING 4/22 TO 13" THEN ADDED INFORMATION REGARDING HIS ROUTING TO THE WARNING AREA. A/C 2 DID NOT READBACK THE INSTRUCTIONS BUT THEN REQUESTED PROGRESSIVE TAXI INSTRUCTIONS. GC ADVISED A/C 2 OF RESTRICTED VISIBILITY ON THE AIRPORT DUE TO GROUND FOG AND ASKED FOR HIS POSITION. A/C 2 REPORTED HE WAS AT LIMA AND NOVEMBER. GC ISSUED A/C 2 NOVEMBER, PAPA, LEFT TURN ON BRAVO AND A GOOD READBACK WAS PROVIDED BY A/C 2. A/C 1 REPORTED READY FOR TAKEOFF AND WAS ADVISED TO STANDBY. LC ISSUED A/C 1 A TAKEOFF CLEARANCE FOR RUNWAY 13. A/C 1 REPLIED "CLEARED AND SWITCHING". A/C 1 BROADCAST "TOWER YOU COPY THAT PLANE CROSSING IN FRONT OF ME"? A/C 1 FURTHER STATED THAT THE PLANE "CROSSED AN ACTIVE RUNWAY AS I WAS ROTATING" AND CAME WITHIN ABOUT 500 FEET. LC ASKED A/C 2 HIS POSITION AND A/C 2 STATED "WE'RE ON ALPHA HEADING TO 22". A/C 2 CROSSED RUNWAY 13 AT TAXIWAY ALPHA WITHOUT APPROVAL . NOTE: FLIGHT STANDARDS INVESTIGATION CONCLUDED FLYOVER VERTICAL ALTITUDE WAS THIRTY-FEET.

    TA/C 2/DHC2 WAS CLEARED FOR TAKEOFF WEST WATERLANE. TA/C 1/C206 CALLED FROM THE FISH AND WILDLIFE COVE, REQUESTING TO TAXI VIA THE SLOW CHANNEL TO THE WEST WATERLANE FOR A NORTHBOUND DEPARTURE. LOCAL CONTROL INSTRUCTED C206 TO TAXI VIA THE SLOW CHANNEL TO THE WEST WATERLANE. HOWEVER LC WAS DISTRACTED, MOOSE WERE OBSERVED INSIDE THE FENCE LINE IN THE VICINITY OF THE GRAVEL STRIP. LC WAS UNAWARE THAT C206 WAS IN THE FISH AND WILDLIFE COVE AND THUS FAILED TO HOLD THE C206 SHORT OF THE WEST WATERLANE FOR THE DEPARTING DHC2. C206 CROSSED THE WEST WATERLANE WITHOUT A CLEARANCE IN ROUTE TO THE SLOW CHANNEL AND IN FRONT OF THE DEPARTING DHC2. C206 ADVISED LC THAT, BASICALLY I WAS IN THE MIDDLE OF THE RUNWAY WHEN HE WENT RIGHT OVER MY HEAD. ADDITIONALLY PILOT OF C206 COMMENTED TO LC, ... GOOD JOB OF PILOTS (DHC2) THEY AVOIDED ME. LOSS OF SEPARATION, TOWER ESTIMATED CLOSEST PROXIMITY WAS 200 FEET VERTICAL.

    A/C 1, C152, DEPARTED WITHOUT A CLEARANCE. THE C152 WAS HOLDING SHORT OF RUNWAY 19R AT TAXIWAY ALPHA 6 AND A/C 2, C210, WAS HOLDING SHORT AT TAXIWAY LIMA. THE C210 CALLED NUMBER ONE READY OF TAKEOFF ON RUNWAY 19R AND WAS TOLD HOLD SHORT. APPROXIMATELY 10 SECONDS LATER THE C152 CALLED NUMBER ONE READY FOR TAKEOFF ON RUNWAY 19R. LOCAL CONTROL DID NOT ACKNOWLEDGE THE C152 AND CLEARED A/C 2 FOR IMMEDIATE TAKEOFF. BOTH AIRCRAFT ACKNOWLEDGED AND THE READBACK WAS GARBLED HOWEVER, THE WHISKEY MIKE PORTION OF THE CALL SIGN WAS AUDIBLE DURING THE READBACK. AS THE C210 WAS ENTERING THE RUNWAY HE ABRUPTLY STOPPED WITH ABOUT 6 FEET OF HIS NOSE OVER THE RUNWAY EDGE. THE C152 SWERVED SLIGHTLY RIGHT WHEN HE NOTICED THE C210 AND CONTINUED THEIR TAKEOFF ROLL. THE CLOSEST LATERAL PROXIMITY WAS 15 FEET LATERAL WHEN THE C152 PASSED IN FRONT OF THE C210. THE C210 HAD ENTERED THE RUNWAY AND IS ESTIMATED TO HAVE CROSSED THE EDGE LINE BY 6 FEET. (PSWTRVS16002)
