PER AIRCRAFT 1'S REQUEST, LOCAL CONTROL 1 (LC1) INSTRUCTED AIRCRAFT 1 TO CONTINUE INBOUND AND CHANGE TO RUNWAY 4R, TRAFFIC WAS HOLDING IN POSITION. AIRCRAFT 1 REPLIED, SWITCHING TO RUNWAY 4R, AND WAS SWITCHED TO THE LOCAL CONTROL 2 (LC2) FREQUENCY. AIRCRAFT 2/P28A WAS IN LUAW STATUS ON RUNWAY 4R. AS AIRCRAFT 1 CONTINUED INBOUND THE PILOT NEVER ESTABLISH COMMUNICATION WITH LC1 OR LC 2, EVEN THOUGH REPEATED ATTEMPTS WERE MADE BY BOTH THE LC1 AND LC2 POSITIONS. AIRCRAFT 1 OVERFLEW AIRCRAFT 2, AND LANDED ON RUNWAY 4R WITH AIRCRAFT 2 STILL IN LUAW POSITION. ACCORDING TO THE FACILITY, AIRCRAFT 1 PASSED OVERHEAD AN ESTIMATED 100-200FT ABOVE AIRCRAFT 2, AND LANDED 2,000FT FURTHER DOWN THE RUNWAY. CONFLICT/FLYOVER EVENT.

AIRCRAFT 2 WAS CLEARED FOR TAKE-OFF ON RY 28R BY LOCAL CONTROL (LC) AND ROLLED FOR DEPARTURE. AIRCRAFT 1 TAXIED ACROSS THE HOLD SHORT LINE AND STOPPED ON THE RY 28R RUNWAY EDGE AS AIRCRAFT 2 PASSED TWY B5 AT 62 FEET AIRBORNE. AIRCRAFT 2 WAS TRAVELLING AT 151 KNOTS WHEN IT JUST BECAME AIRBORNE AT TWY B5. AIRCRAFT 1 WAS TRAVELLING AT 18 KNOTS WITH ITS NOSE OVER THE SOUTH RUNWAY EDGE WHEN AIRCRAFT 2 PASSED TWY B5. AIRCRAFT 1 THEN MADE A 180 AND EXITED THE RY ON TWY B5. THE CLOSEST PROXIMITY BETWEEN THE AIRCRAFT WAS 133 FEET. AIRCRAFT 2 OVERFLEW AIRCRAFT 1 BY 62 FEET. GC THEN INSTRUCTED AIRCRAFT 1 TO CROSS RY 28R AT TWY B5 AND CONTINUE VIA TWY A TO THE RAMP. AIRCRAFT 1 CROSSED RY 28R STRAIT AHEAD ON B5 AND TAXIED INTO THE GRASS ON THE OTHER SIDE. AIRCRAFT 1 WAS ABLE TO TAXI OUT OF THE GRASS BACK ON TO THE RY AND JOIN TWY A3 TO EXIT THE RY. RY 28R AND ASSOCIATED TAXIWAYS WERE RECENTLY REPAVED OR RELOCATED ALONG WITH ALL TAXIWAYS RENAMED. AIRCRAFT 1 PASSED THE 'NEXT TAXIWAY' CONTINUING ON THE RWY, ATC ADVISED AIRCRAFT 1 NO DELAY TO NEXT INTERSECTION. AIRCRAFT 1 ACKNOWLEDGES. ATC CLEARED AIRCRAFT 2 FOR TAKE-OFF AS AIRCRAFT 1 WAS CLEARING THE RWY AT A7. AIRCRAFT 1 CAME TO A STOP AFTER CLEARING THE RWY EDGE LINES BUT PRIOR TO CROSSING THE HOLD BAR. ATC INSTRUCTED AIRCRAFT 1 TO CONTINUE TAXI FORWARD PAST THE HOLD BARS. AIRCRAFT 1 GAVE A GOOD READ BACK. AIRCRAFT 1 MADE A 180 DEGREE TURN BACK TOWARDS THE RUNWAY. ATC INSTRUCTED AIRCRAFT 1 TO MAKE A 180 AND JOIN ALPHA, INCLUDING A WARNING NOT TO HEAD BACK TOWARDS THE RUNWAY. SHORTLY THEREAFTER ATC ADVISES AIRCRAFT 1 TO STOP! AIRCRAFT 1 READS BACK STOPPING. ACCORDING TO FACILITY NOTES AIRCRAFT 1 WAS STOPPED 15-20 FEET FROM THE NORTH SIDE RWY26 EDGE LINES ABEAM A7, THE POINT OF AIRCRAFT 2'S ROTATION. AFTER AIRCRAFT 2 DEPARTED, ATC PROGRESSIVELY GUIDED AIRCRAFT 1 OFF THE RUNWAY. ESTIMATED CLOSEST PROXIMITY IS 25 FEET, WINGTIP OF B767 AND AIRCRAFT 1. NIGHT TIME OPERATION.

AIRCRAFT 1 CONTACTED ROCHESTER TOWER TEN MILES TO THE NORTHWEST VFR INBOUND FOR LANDING. TOWER INSTRUCTED AIRCRAFT 1 TO CONTINUE INBOUND FOR A LEFT DOWNWIND RUNWAY 2. PILOT RESPONDED WITH CALL SIGN ONLY. TOWER CLEARED AIRCRAFT 1 TO LAND ON RUNWAY 2 BUT DID NOT RECEIVE A RESPONSE. AIRCRAFT 1 APPEARED TO ENTERED A LEFT DOWNWIND FOR RUNWAY 31. TOWER ASKED AIRCRAFT 1 TO SAY INTENTIONS BUT DID NOT GET A RESPONSE. TOWER ATTEMPTED SEVERAL TIMES TO REESTABLISH COMMUNICATIONS WITH AIRCRAFT 1 BUT WAS UNSUCCESSFUL. TOWER OBSERVED AIRCRAFT 1 TURN FINAL FOR RUNWAY 31 AND BROADCASTED TO AIRCRAFT 1, RUNWAY 31 WAS CLOSED WITH PERSONNEL AND EQUIPMENT AND TO TURN RIGHT FOR RUNWAY 2 BUT THERE WAS STILL NO RESPONSE FROM AIRCRAFT 1. LOCAL CONTROL ISSUED GO-AROUND INSTRUCTIONS TO AIRCRAFT 1 SEVERAL TIMES. TOWER LOCAL AND GROUND POSITIONS BOTH GAVE FLASHING RED LIGHT GUN SIGNALS. TOWER ALSO CONTACTED AIRPORT 11 AND ADVISED THEM TO GET ALL VEHICLES OFF THE RUNWAY ANTICIPATING THAT AIRCRAFT 1 WAS GOING TO LAND ON THE CLOSED RUNWAY. AIRCRAFT 1 CONTINUED TO FLY OVER THE TOP OF THE WORKERS WHO WERE ON RY31 BETWEEN THE APPROACH END AND TAXIWAY DELTA AND THEN LANDED ON RUNWAY 31 JUST PAST TAXIWAY DELTA. CLOSEST PROXIMITY WAS 100 FEET VERTICAL AND 75 FEET LATERAL.

GC INSTRUCTED AIRCRAFT 1 TO CONTINUE TAXI ON RUNWAY15, CROSS RUNWAY 19, TAXIWAY MIKE TO RUNWAY 22 HOLDING BAY. LC THEN CLEARED AIRCRAFT 2 FOR TAKEOFF ON RUNWAY 19, CONFLICTING WITH AIRCRAFT 1 CROSSING RUNWAY 19. AIRCRAFT 2 BEGAN TAKEOFF ROLL AND WAS ACCELERATING THROUGH 60 KNOTS WHEN THEY REPORTED THAT THEY WERE ABORTING TAKEOFF. ASDE ALARM WAS ACTIVATED AND THE CLOSET PROXIMITY BETWEEN THE AIRCRAFT WAS APPROXIMATELY 730 FEET. LOSS OF SEPARATION VALIDATED.

    PILOT DEVIATION, RUNWAY INCURSION WITH CONFLICT. AIRCRAFT 1, PA32, DEPARTED FROM RUNWAY 34 WITHOUT A CLEARANCE AND OVERFLEW AIRCRAFT 2, P28A, WHICH WAS CLEARED TO CROSS RUNWAY 34. THE FACILITY REPORTED THAT AIRCRAFT 1 OVERFLEW AIRCRAFT 2 BY AN ESTIMATED 100 FEET. TRAINING WAS TAKEN PLACE AT THE LOCAL CONTROL (LC) POSITION. AIRCRAFT 1 WAS HOLDING SHORT OF RUNWAY 34 WHEN THE CONTROLLER INSTRUCTED THEM TO LUAW AND ADVISED THE PILOT THAT AIRCRAFT 2 WOULD BE CROSSING DOWNFIELD. THE CONTROLLER THEN INSTRUCTED AIRCRAFT 2 TO CROSS RUNWAY 34 AT TAXIWAY D WITH NO DELAY AND ADVISED THE PILOT THAT AIRCRAFT 1 WOULD BE HOLDING IN POSITION ON RUNWAY 34. THE FACILITY REPORTED THAT THE LC CONTROLLER AND THE OJTI OBSERVED AIRCRAFT 2 BEGIN THE CROSSING, THEN TURNED THEIR ATTENTION TO SCANNING AND SEQUENCING AIRCRAFT IN THE WEST TRAFFIC PATTERN. CONCURRENTLY, AIRCRAFT 1 HAD BEGUN TAKEOFF ROLL WITHOUT A DEPARTURE CLEARANCE. IT WAS REPORTED BY THE TIME THE LC CONTROLLER AND THE OJTI REALIZED THAT AIRCRAFT 1 HAD BEGUN TAKEOFF ROLL, THE AIRCRAFT HAD ALREADY ROTATED AND BECOME AIRBORNE. THE FACILITY REPORTED THAT AIRCRAFT 2 WAS APPROXIMATELY HALFWAY ACROSS RUNWAY 34 WHEN AIRCRAFT 1 OVERFLEW THE AIRCRAFT BY APPROXIMATELY 100 FEET. CLOSEST PROXIMITY WAS ZERO FEET LATERAL AND 100 FEET VERTICAL.
