Incident Description
After departing Miami and climbing through 10,900 feet, the number 3 right main landing gear tire exploded. The explosion caused massive damage to the hydraulic lines of systems A and B, resulting in the loss of both hydraulic systems. The aircraft returned to Miami. The crew attempted to extend the right main landing gear using emergency manual procedures but was unable to do so. The aircraft landed with the right main gear retracted. Upon touchdown, the left main and nose landing gears collapsed and separated.
Tire Examination Findings
Examination of the number 3 tire revealed massive ply separations. These separations originated at the toe bead, extended down the sidewall, and continued circumferentially around the crown at the 10th to 13th ply layers. Inspection of the area where the separation met the bead bundles identified a location where the bead seat had been sanded and smoothed. The abrasion had removed the outer chafer strip and several cover plies, exposing the plies covering the toe bead. The bead seat and the cords wrapped around the toe bead had been subjected to excessive heat for an extended period, leading to severe deterioration of the bead seat area.
