What happened
During the initial climb phase following departure from Congonhas Airport in São Paulo, the aircraft experienced significant vibrations originating from the left engine. In response to this malfunction, the crew performed an engine shutdown and feathered the propeller in an attempt to return to the airfield.
As the flight continued toward the airport, the right engine also encountered technical difficulties. During the subsequent approach, the pilot extended the landing gear; however, this action resulted in a loss of altitude. The aircraft subsequently struck various obstacles and crashed in the vicinity of a soccer field.
The accident resulted in four fatalities (the two pilots and two passengers), with 18 injuries reported among the other occupants, several of which were critical. Tragically, one individual who initially survived the impact later succumbed to their injuries.
Findings
Investigations determined that the left engine failed during the climb because of a failure in the accessory drive. The right engine also malfunctioned, a situation attributed to inadequate maintenance practices by the operator, which did not adhere to the manufacturer's prescribed procedures. Additionally, the decision by the crew to lower the undercarriage contributed to the loss of airspeed and subsequent stall.
