On October 3, 2021, a Robinson R4/4, registration RA-04172, was scheduled to perform nine flights between the 'Condor' landing site and various locations within airspace classes C and G. The pilot-in-command was operating the aircraft under a power of attorney from the owner. While the pilot had obtained necessary flight permissions, the pre-flight preparation of the helicopter was not documented in the aircraft logbook.
During the day's operations, a total of ten flights were performed. Analysis of ADS-B and GPS data indicated that one flight, occurring between 14:38 and 14:52, was not reported to air traffic control authorities. Following the ninth flight, the pilot reported to the Myachkovo landing site dispatcher that the aircraft had landed at the 'Condor' site.
After two passengers boarded the aircraft, the pilot reported a successful test hover and takeoff at 16:24:54. However, the aircraft subsequently crashed in the area of the Lytkarino settlement in the Moscow region, resulting in three fatalities.
Post-accident investigations focused on the engine and electrical components. The Lycoming O-540-F1B5 engine's left and right magnetos were found to be in working order prior to the impact. While the generator showed mechanical damage to its casing, investigators found no evidence of malfunction preceding the collision with the ground.
