What happened
On a night MEDEVAC mission from Gods Lake Narrows, Manitoba, a Fairchild SA226TC operated by Perimeter Airlines was conducting a descent toward Shamattawa. The flight, which included two pilots and a flight nurse, was attempting a visual approach to Runway 01 after descending through an overcast layer to approximately 3000 feet above sea level. During the final approach, the crew determined the aircraft was traveling at an excessive speed and was positioned too high, leading them to initiate a missed approach procedure.
At 23:33, roughly 30 seconds after increasing engine power, the aircraft struck trees located approximately 2600 feet from the end of the runway, slightly left of the centerline. The impact caused the Fairchild SA226TC to break apart over a debris field spanning roughly 850 feet. While the area behind the cockpit maintained some structural form, the rest of the airframe suffered significant damage. A fire broke out following the crash, though it remained limited to the wings that had detached from the main wreckage.
Findings
The accident resulted in two fatalities and one serious injury. The two pilots were killed upon impact, while the flight nurse survived the crash with serious injuries and managed to exit the cabin. Data from the cockpit voice recorder indicated that the crew maintained control of the aircraft and voiced no immediate concerns regarding the flight's status prior to the collision with the trees.
