What happened
During a routine local training mission operating out of Rio de Janeiro-Galeão Airport, the flight crew engaged in a simulated emergency procedure. While on final approach to the runway, the instructor pilot intentionally deactivated engine number four to mimic an in-flight engine failure. During this maneuver, the student pilot lost command of the aircraft type (not specified), leading to an aerodynamic stall.
The aircraft subsequently struck the runway surface before veering away from the active pavement and coming to a halt. Although the incident resulted in significant damage to the airframe, all four crew members were evacuated from the wreckage without injury.
Findings
Investigations into the accident identified several contributing elements related to the execution of the training maneuver. The instructor's decision-making was characterized by an overconfidence that led to a lapse in judgment during the simulation. This was compounded by the trainee pilot performing improper maneuvers and maintaining an incorrect configuration for the approach.
