FOUR MINUTES LATER PRM1 CALLED FOR IFR DEPARTURE ON RUNWAY 13 AND WAS CLEARED FOR TAKEOFF. ON DEPARTURE PRM1 ROTATED AT APPROXIMATELY THE INTERSECTION WITH RUNWAY 18/36/TAXIWAY ALPHA 3. VEHICLE REPORTED THEY WERE POSITIONED ON RUNWAY 13 APPROXIMATELY 1000 FEET FROM THE DEPARTURE END. TOWER REPORTED PRM1 FLEW OVER MOBILE 60 BY APPROXIMATELY 200 FEET. RADAR INDICATES PRM1 FLEW OVER MOBILE 60 BY APPROXIMATELY 100 FEET.

    C550 HAD BEEN CLEARED TO LAND ON RUNWAY 34. WHEN C550 WAS APPROXIMATELY 4 NM FINAL, N6265T/C172 CALLED LOCAL CONTROL (LC), REQUESTING DEPARTURE FROM THE END OF RUNWAY 34. LC ISSUED RUNWAY HEADING, AND CLEARED THE C172 FOR TAKEOFF. (LC DID NOT ISSUE THE ARRIVAL TRAFFIC TO THE DEPARTING C172, NOR THE DEPARTING TRAFFIC TO THE ARRIVING C550). LC ISSUED TRAFFIC BETWEEN ANOTHER CESSNA AND LJ45 NORTH OF THE AIRFIELD. (TWO CALLS TO ESTABLISH VISUAL CONTACT). LC DID NOT RECOGNIZE THE CLOSURE BETWEEN THE DEPARTING C172 AND THE ARRIVING C550, UNTIL THE C550 REPORTED ON SHORT FINAL THAT HE WAS EXECUTING A MISSED APPROACH. WHEN LC QUERIED WHY THE C550 WAS GOING-AROUND, THE PILOT REPORTED THAT AN AIRCRAFT PULLED OUT IN FRONT OF HIM ON THE RUNWAY, AT THE APPROACH END. LC INSTRUCTED THE C550 TO ENTER RIGHT TRAFFIC FOR RUNWAY 34. THE RADAR REPLAY SHOWED THE C550 AT 800 FT MSL ON SHORT FINAL (RUNWAY 34 ELEVATION 645 FT MSL) UNTIL APPROXIMATELY MIDFIELD ON HIS GO-AROUND. THE C550 THEN INDICATED 900 FT. MSL TO THE DEPARTURE END OF THE RUNWAY BEFORE COMMENCING A TURN. THE C550 PILOT INDICATED HE WAS APPROXIMATELY 200 FEET AGL WHEN HE STARTED THE CLIMB AND ESTIMATES HE OVERFLEW THE C172 BETWEEN 300 AND 400 FEET.

    LOCAL CONTROL (LC) CLEARED C172 TO LAND ON RUNWAY 23. DURING THE C172'S LANDING ROLL, LC AND THE C172 OBSERVED PA32 FLY DIRECTLY OVER THE TOP OF THE C172 AND LAND ON RUNWAY 12R WITHOUT A CLEARANCE OR CONTACTING THE TOWER. PA32 TURNED OFF OF RUNWAY 12R ONTO TAXIWAY BRAVO. PA32 CALLED GROUND CONTROL (GC) AND REPORTED HE WAS HAVING RADIO PROBLEMS AND DID NOT CONTACT THE TOWER INBOUND. GC ASKED IF THE PA32 FOLLOWED NORDO PROCEDURES BY ENTERING DOWNWIND AND ROCKING HIS WINGS. PA32 REPLIED HE DID NOT. (THE PA32 APPROACHED THE AIRPORT FROM THE SOUTHWEST AND ENTERED A TIGHT, RIGHT BASE TO RUNWAY 12R). TOWER ESTIMATED THE CLOSEST PROXIMITY AT 100 FT VERTICAL, AND IN A SUBSEQUENT PHONE CALL, THE PILOT OF THE C172 ESTIMATED THE DISTANCE AT 50 FT VERTICAL.

    C210 LANDED RUNWAY 35L AFTER BEING TOLD TO GO AROUND (TWICE) BY THE TOWER DUE INSUFFICIENT SEPARATION FROM PRECEDING AIRCRAFT, C172, EXECUTING A STOP AND GO MANEUVER . BOTH GO AROUND INSTRUCTIONS WERE ISSUED PRIOR TO THE C210 CROSSING RUNWAY 35L THRESHOLD. C210 LANDED ON RUNWAY 35L AFTER ANNOUNCING BINGO FUEL. TOWER TOLD THE C172 TO START AN IMMEDIATE TAKE-OFF ROLL PRIOR TO THE C210 LANDING. THE AGGRESSIVE BRAKING BY THE C210 PILOT LEAD TO THE AIRCRAFT MAKING A 180 DEGREE TURN ON THE RUNWAY PAST TAXIWAY ALF 3 RESULTING IN A BLOWN RIGHT MAIN GEAR TIRE. CLOSEST PROXIMITY REPORTED BY THE TOWER WAS 300 FEET HORIZONTAL AND ZERO FEET VERTICAL.

    GROUND CONTROL EAST (GC-E) RECEIVED APPROVAL FROM LOCAL CONTROL EAST (LC-E) TO CROSS RUNWAY 18L AT TAXIWAY MIKE FOR /CRJ7, ASSIGNED RUNWAY 18L FOR DEPARTURE VIA TAXIWAYS MIKE AND DELTA. BUT AS THE CRJ7 BEGAN CROSSING, LC-E ISSUED A TAKEOFF CLEARANCE TO CRJ2 THAT WAS IN POSITION ON THE RUNWAY 18L NUMBERS. THE ASDE-X ALERTED THE CONFLICT, BUT THE LC-E CONTROLLER DETECTED THAT THE CRJ2 HAD ALREADY INITIATED THE ABORTED TAKEOFF AND DID NOT CANCEL THE TAKEOFF CLEARANCE. THE CRJ2 PILOT INDICATED THAT HE HAD REACHED 40-50 KNOTS AT THE TIME OF THE REJECTED TAKEOFF AND DID NOT CONSIDER HIS ACTION TO BE A HIGH SPEED ABORT. THE CLOSEST PROXIMITY BETWEEN THE 2 AIRCRAFT WAS CALCULATED TO BE 414 FEET ON THE RUNWAY SURFACE. SUBSEQUENTLY, THE CRJ2 TAXIED BACK TO RUNWAY 18L VIA A RIGHT TURN ONTO RUNWAY 5, FOLLOWED BY TAXIWAYS ROMEO, MIKE AND CHARLIE.

    C208 VFR INBOUND TO RUNWAY 4L REQUESTED RUNWAY 8L. PILOT WAS ISSUED LANDING CLEARANCE ON RUNWAY 4L AND THE PILOT READ-BACK WAS CORRECT.. PILOT WAS RE-CLEARED TO LAND ON RUNWAY 4L WITH CORRECT PILOT ACKNOWLEDGEMENT. ASDE-X ALERT ALARMED, WARNING RUNWAY 4R CLOSED, AS THE AIRCRAFT WAS ROLLING OUT ON RUNWAY 4R WHICH WAS CLOSED FOR MAINTENANCE AND OCCUPIED BY PERSONNEL AND EQUIPMENT. AIRCRAFT CAME TO A STOP APPROXIMATELY 1000 FEET FROM THE APPROACH END OF RUNWAY 4R AND APPROXIMATELY 100 FEET FROM AN ARRESTING BARRIER WITH ASSOCIATED PERSONNEL AND EQUIPMENT ON AND ADJACENT TO THE RUNWAY. PILOT STATED HE HAD LOST SITUATIONAL AWARENESS TURNING LEFT BASE AND LANDED ON THE WRONG RUNWAY. LOSS OF SURFACE SEPARATION DUE TO VEHICLES AND PERSONNEL ON CLOSED RUNWAY 4R AS AIRCRAFT LANDED.

    C172 WAS HOLDING SHORT OF RUNWAY 7L AND PA28 WAS INBOUND AND CLEARED FOR A TOUCH AND GO ON RUNWAY 7L. LOCAL CONTROL (LC) INSTRUCTED C172 TO LINE UP AND WAIT ON RUNWAY 7 LEFT. READ BACK WAS CORRECT. IMMEDIATELY AFTER THE C172 READ BACK THE INSTRUCTION, PA28 INITIATED A GO AROUND. SHORTLY AFTER THIS, PA28 ADVISED THE LC HE HAD CLEARED AN AIRCRAFT ON THE RUNWAY BEFORE HE HAD PASSED HIM. THE RISK ANALYSIS PANEL AND LANDING AIRCRAFT PILOT ESTIMATED CLOSEST PROXIMITY AS 100 TO 200 FOOT OVERFLIGHT.

    C750 REQUESTED TAXI FROM THE RAMP AT TAXIWAY ALPHA 3 FOR DEPARTURE. INITIAL TAXI INSTRUCTIONS FROM GROUND CONTROL (GC) WERE TO CROSS RUNWAY 25L AT ALPHA 3 WITHOUT DELAY, TO ALPHA AND "MORE TO FOLLOW". THE C750 THEN ADVISED THAT HE WAS NOT READY TO TAXI. THE SECOND TAXI CLEARANCE WAS TO CROSS RUNWAY 25L, VIA TAXIWAY ALFA AND MIKE HOLD SHORT OF RUNWAY 19R. (GC DID NOT ISSUE A DEPARTURE RUNWAY ASSIGNMENT). READ BACK WAS CORRECT. GC SUBSEQUENTLY INSTRUCTED THE C750 TO CROSS RUNWAY 19R ON TAXIWAY MIKE AND HOLD SHORT OF RUNWAY 19L. READ BACK WAS CORRECT. GC TWICE INSTRUCTED THE C750 TO CROSS RUNWAY 19L AND TO MONITOR LOCAL CONTROL (LC). C750 DID NOT ACKNOWLEDGE. (THE C750 DID NOT CROSS RUNWAY 19L ON MIKE). THE C750 CALLED LC READY FOR TAKEOFF, WHILE STILL HOLDING SHORT OF RUNWAY 19L. THE PILOT ASKED HOW MUCH RUNWAY WAS FROM HIS LOCATION TO THE END, AND ADVISED THAT HE NEEDED 5,000 FEET. LC RESPONDED THAT THE C750 HAD ABOUT THAT MUCH DISTANCE REMAINING. (RUNWAY 19L IS 4,183 FEET LONG, FULL-LENGTH, WITH 2,900 FEET AVAILABLE FROM TAXIWAY MIKE). THE C750 ADVISED HE WAS READY TO GO. LC CLEARED C750 FOR TAKEOFF RUNWAY 25L. THE C750 READ BACK TAKEOFF CLEARANCE ON RUNWAY 25L "AT MIKE", BUT TURNED RIGHT ON RUNWAY 19L AND BEGAN TAKEOFF ACCELERATION. LC OBSERVED THE WRONG-RUNWAY DEPARTURE DEVELOPING, AND CANCELLED THE C750'S TAKEOFF CLEARANCE. (ASDE-X ANALYSIS INDICATES THE AIRCRAFT REACHED A SPEED OF NEARLY 90 KNOTS BEFORE SUCCESSFULLY ABORTING). AFTER ABORTING TAKEOFF, THE C750 STATED "MY APOLOGIES, 19R. I MISUNDERSTOOD. I THOUGHT IT WAS 19R WE WERE SAYING" INDICATING HE THOUGHT HE WAS DEPARTING ON RUNWAY 19R ALTHOUGH THE AIRCRAFT WAS INSTEAD ON 19L.
