ATC DID NOT PROVIDE RUNWAY SEPARATION BETWEEN AIRCRAFT 1 AND AIRCRAFT 2 ON CONVERGING RUNWAYS. AIRCRAFT 1/CL35 WAS ON 9 MILE FINAL AND CLEARED TO LAND ON RUNWAY 30. AIRCRAFT 2/SLG2 DEPARTED RUNWAY 26L AND INFORMED THE TOWER OF A ROUGH RUNNING ENGINE AND THE NEED TO RETURN TO LAND. LOCAL CONTROL INSTRUCTED AIRCRAFT 2 TO ENTER A LEFT DOWNWIND FOR RUNWAY 26L AND ASKED AIRCRAFT 2 IF THEY REQUIRED ANY ADDITIONAL SERVICES. AIRCRAFT 2 STATED THAT THE ISSUE WAS NOT AN EMERGENCY AND THAT NO ASSISTANCE WAS NEEDED. THE LC THEN CLEARED AIRCRAFT 2 TO LAND ON RUNWAY 26L AND ADVISED THEM THAT THEY WOULD BE IN FRONT OF TRAFFIC INBOUND TO RUNWAY 30. THE LC THEN INFORMED AIRCRAFT 1 THAT TRAFFIC WAS A LIGHT SPORT LANDING RUNWAY 26L PRIOR TO THEIR ARRIVAL. AIRCRAFT 1 ADVISED THAT THEY WOULD REDUCE THEIR AIRSPEED. THE LOCAL CONTROLLER ATTEMPTED TO SEQUENCE AIRCRAFT 2 IN FRONT OF AIRCRAFT 1 BY INSTRUCTING AIRCRAFT 1 TO MAKE S TURNS AND ASKING AIRCRAFT 2 TO TURN TOWARDS THE RUNWAY. HOWEVER, AIRCRAFT 2 DID NOT REPLY BECAUSE THE LC USED AN INCORRECT TRUNCATED CALLSIGN. AIRCRAFT 1 WAS OBSERVED INITIATING AN S-TURN TO THE RIGHT ON 1.2 MILE FINAL. AS AIRCRAFT 1 APPROACHED THE RUNWAY 30 DISPLACED THRESHOLD, INDICATING 130 KTS, THE LC ATTEMPTED TO INSTRUCT AIRCRAFT 2 TO START A CLIMB AND GO AROUND. NO REPLY WAS RECEIVED FROM AIRCRAFT 2, AS THE LC USED AN INCORRECT CALLSIGN. AFTER NOT RECEIVING A RESPONSE, THE LC THEN ADVISED AIRCRAFT 2 TO CONTINUE TO LAND RUNWAY 26L. AGAIN, AIRCRAFT 2 DID NOT REPLY AS AN INCORRECT CALLSIGN WAS USED BY THE LOCAL CONTROLLER. BOTH AIRCRAFT CONTINUED TO LAND ON THEIR ASSIGNED RUNWAYS. AIRCRAFT 1 PASSED THROUGH THE EXTENDED FINAL APPROACH COURSE TO RUNWAY 26L AS AIRCRAFT 2 WAS INSIDE THE LATERAL DIMENSIONS OF THE RUNWAY 30 RSA. QA ESTIMATES THAT AIRCRAFT 1 PROCEEDED AIRCRAFT 2 THROUGH THE RUNWAY INTERSECTION WITH APPROXIMATELY 50FT LATERAL AND LESS THAN 50FT VERTICAL SEPARATION.

WHEN AIRCRAFT 2 WAS ON A 1/2 MILE FINAL TO RUNWAY 27R, THE LOCAL CONTROLLER CLEARED AIRCRAFT 1/PA28 FOR TAKEOFF FROM RUNWAY 27R AT TAXIWAY R. AIRCRAFT 1 CROSSED THE RUNWAY 27R HOLD BAR AT TAXIWAY R AFTER AIRCRAFT 2 HAD CROSSED RUNWAY 27R LANDING THRESHOLD. AIRCRAFT 1 WAS PAST THE RWY 27R RUNWAY EDGE LINE AS AIRCRAFT 2 PASSED TAXIWAY R DURING LANDING ROLL. THE TAKEOFF CLEARANCE FOR AIRCRAFT 1 WAS CANCELLED AFTER THIS INCIDENT HAD OCCURRED.. AFTER LANDING, AIRCRAFT 2 ADVISED LC, "SOMEONE WAS CROSSING THE RUNWAY WHEN WE WERE LANDING." A LOSS OF SEPARATION OCCURRED BETWEEN AIRCRAFT 2 AND AIRCRAFT 1. CLOSEST PROXIMITY APPROXIMATELY 75 FEET.

GC PREVIOUSLY INSTRUCTED AIRCRAFT 2/C560 TO FOLLOW AN UNINVOLVED CL30 HOLDING SHORT OF RUNWAY 13C ON TAXIWAY W. A POSITION RELIEF BRIEFING WAS CONDUCTED TO COMBINE GC WITH CD WHERE THE GROUND CONTROLLER REFERENCED APPROVED COORDINATION TO CROSS THE RUNWAY 13R APPROACH HOLD. AIRCRAFT 1 OVERFLEW AIRCRAFT 2 INSIDE THE RPZ. CLOSEST PROXIMITY WAS 35 FEET VERTICAL AND 56 FEET LATERAL.

LOCAL CONTROL CLEARED AIRCRAFT 2 FOR A SHORT APPROACH TO RUNWAY 7R. THE PILOT READ BACK WAS CORRECT. WITH AIRCRAFT 2 TURNING BASE TO SHORT FINAL, THE LC OBSERVED AIRCRAFT 1 BEGIN TO MOVE FORWARD AND ENTER RUNWAY 7R. THE LC THEN INSTRUCTED AIRCRAFT 1 TO HOLD POSITION. AIRCRAFT 1 WAS OBSERVED HOLDING POSITION, BUT DID NOT READ BACK. NO INSTRUCTION TO GO-AROUND WAS GIVEN TO AIRCRAFT 2. AIRCRAFT 2 OVERFLEW AIRCRAFT 1 BY APPROXIMATELY 60 FEET AGL AND TOUCHED DOWN AT THE DISPLACED THRESHOLD APPROXIMATELY 900 FEET FROM THE RUNWAY END.

WHEN AIRCRAFT 2 WAS INSIDE A HALF-MILE FINAL TO RUNWAY 25 AND AIRCRAFT 1 WAS ON 2.5 MILE FINAL TO RUNWAY 7, AIRCRAFT 2 ASKED IF THEY WERE CLEARED TO LAND AND THE CONTROLLER RESPONDED AFFIRMATIVE. AIRCRAFT 2 LANDED RUNWAY 25 AND WAS INSTRUCTED TO TURN RIGHT AT TAXIWAY K, BUT THEY WERE UNABLE. THE CONTROLLER ISSUED AIRCRAFT 1 A GO AROUND INSIDE A HALF-MILE FINAL. AIRCRAFT 1 OVERFLEW AIRCRAFT 2 ON RUNWAY 7. CLOSEST PROXIMITY IS ESTIMATED BETWEEN 300-500 FEET VERTICAL.

LC VERIFIED TURN RIGHT ON E HOLD SHORT OF RUNWAY 4L. THE PILOT READ BACK WAS CORRECT. THE LOCAL CONTROLLER CLEARED AIRCRAFT 2/BE9L FOR TAKEOFF ON RUNWAY 4L. AS AIRCRAFT 1 BEGAN CROSSING THE RUNWAY 4L HOLD BAR, THE LOCAL CONTROLLER VERIFIED AIRCRAFT 1 WAS HOLDING SHORT OF RUNWAY 4L. THE PILOT RESPONSE WAS UNINTELLIGIBLE. LC INSTRUCTED AIRCRAFT 1 TO STOP. AIRCRAFT 1 ENTERED RUNWAY 4L AT TAXIWAY E AS AIRCRAFT 2 WAS PASSING THROUGH THE RUNWAY 4L/TAXIWAY E INTERSECTION ON DEPARTURE ROLE. ASDE-X REPLAY ESTIMATES CLOSEST PROXIMITY AT 54 FEET OR LESS LATERALLY.

AIRCRAFT 2 ENTERED RUNWAY 36R AND CONDUCTED A ROLLING DEPARTURE DUE TO THEIR WEIGHT. AIRCRAFT 1 LANDED AND ADVISED LC THAT THE AIRCRAFT WAS ON THE RUNWAY WHEN THEY LANDED. THE PILOT OF AIRCRAFT 1 LATER REPORTED THEY HAD TO USE GREATER THAN NORMAL BRAKING PRESSURE TO AVOID CATCHING THE HELICOPTER. THE CLOSEST ESTIMATED PROXIMITY BETWEEN WAS APPROXIMATELY 1042 FEET LATERAL.
