Incident Overview
On January 16, 2020, at 02:12 UTC, a Boeing 787-8 aircraft (registration SP-LRG) operating a flight from Tokyo Narita International Airport (RJAA) to Warsaw Chopin Airport (EPWA) experienced a surge indication on the No. 2 engine. The event was displayed on the Engine Electronic Controller (EEC) screen in the cockpit.
Crew Actions
After the surge subsided, the flight crew decided to continue the flight to the intended destination. The aircraft landed at Warsaw Chopin Airport without further incident.
Investigation Findings
The investigation determined that the surge was caused by the development of excessive clearance in the mechanical linkage of the Variable Stator Vanes (VSV) control system for the high-pressure compressor.
Safety Actions
The aircraft operator, Polskie Linie Lotnicze LOT S.A., recorded the event in its SMS database, classified it according to internal criteria, and included it in safety performance indicators as per its Safety Management Manual. No additional safety recommendations were issued by the investigation authority.
Cause
The Polish State Commission for Aircraft Accident Investigation (PKBWL) identified the cause as the creation of clearance on the mechanical connection of the VSV control system of the high-pressure compressor.
