What happened
During a nighttime approach to Tokyo-Haneda Airport under visual flight rules (VFR), a three-engine aircraft lost control and plunged into Tokyo Bay. The incident occurred at an altitude of approximately 2,000 feet in favorable weather conditions. The impact took place roughly 12 km southeast of the airport, causing the aircraft to disintegrate upon hitting the water.
All 133 occupants perished in the accident. Following the crash, recovery efforts located significant amounts of debris, including the tail section, which settled on the seabed at a depth of 20 meters. Prior to the descent and impact, the crew had maintained contact with air traffic control without reporting any mechanical difficulties or operational anomalies.
Findings
Official investigations were unable to establish a definitive cause for the loss of control. Investigators found no evidence of engine failure, instrument malfunctions, or explosions occurring before the aircraft struck the water. The pilot remained in communication with ATC during the final approach phase and had not signaled any distress or technical issues prior to the disappearance from radar.
