A CANADAIR CRJ7 WAS LUAW ON RUNWAY 22L WHEN A EMBRAER E145 WAS CLEARED TO LAND ON RUNWAY 28. THE E145 FLEW OVER THE CRJ7 STILL HOLDING ON RUNWAY 22L AT AN ESTIMATED 80 FEET VERTICAL AND LANDED RUNWAY 28. CLOSEST HORIZONTAL PROXIMITY IS UNKNOWN. RUNWAY 22L THRESHOLD TO RUNWAY 28 THRESHOLD HORIZONTAL DISTANCE IS APPROXIMATELY 800 FEET.

AWAITING ADDITIONAL INFORMATION. A BEECH BE33 WAS INSTRUCTED TO CONTINUE ON THE ILS RUNWAY 20L APPROACH AND WAS NOT ISSUED A CLEARANCE TO LAND. A CIRRUS SR22 WAS VFR AND CLEARED FOR A TOUCH AND GO ON INTERSECTING RUNWAY 9. THE BE33 LANDED WITHOUT A LANDING CLEARANCE ON RUNWAY 20L THUS CONFLICTING WITH THE SR22 EXECUTING A TOUCH AND GO ON RUNWAY 9. THE BE33 PILOT STATED THAT AS HE WAS ON HIS LANDING ROLL WHEN HE OBSERVED THE SR22 PASSING IN FRONT OF HIM AT THE INTERSECTION OF THE TWO RUNWAYS. THE SR22 PILOT STATED THAT HE TOUCHED DOWN ON RUNWAY 9 AND WAS APPROXIMATELY 50 FEET FROM THE RUNWAY INTERSECTIONS WHEN HE NOTICED THE BE33 APPROACHING THE RUNWAY INTERSECTION AND APPROXIMATELY 100 FEET FROM THE 9/20L INTERSECTION. AT THAT POINT THE SR22 PULLED UP AND STATED THE BE33 WAS OFF HIS LEFT SIDE 50-75 FEET LATERAL AND ESTIMATED BEING AT 20-30 FEET VERTICAL AS THEY WENT THROUGH THE INTERSECTION. CLOSEST HORIZONTAL PROXIMITY WAS 50-75 FEET LATERAL AND 20-30 FEET VERTICAL (PILOT ESTIMATE). THE BE33 CONTINUED THROUGH THE INTERSECTION AFTER THE SR22 PASSED THE 9/20L INTERSECTION.

A BELL B407 (HELICOPTER) WAS INSTRUCTED TO TAXI UP TO AND HOLD SHORT OF RUNWAY 16. PILOT READ BACK THE HOLD SHORT. A GULFSTREAM GLF4 LANDED AND WAS ON ROLL OUT RUNWAY 16 WHEN THE B407 CONTINUED AIR TAXI ACROSS RUNWAY 16 ON TAXIWAY FOXTROT WITHOUT A CLEARANCE. THERE WAS NO FLYOVER AND THE GLF4 DID NOT TAKE ANY EVASIVE ACTIONS. CLOSEST PROXIMITY REPORTED WAS 75 FEET VERTICAL AND 100 FEET LATERAL. DISTANCE FROM APPROACH END 16 TO FOXTROT IS 3,000 FEET.

LOCAL 1(LC1) WAS DEALING WITH STUCK MICROPHONE ON PRIMARY TOWER FREQUENCY (REPORTED TO LAST ONE MINUTE 10 SECONDS). LC1 THEN OBSERVED A CESSNA C172 DEPARTING ON RUNWAY 10 FULL LENGTH FROM C1 WITHOUT CLEARANCE THUS CONFLICTING WITH A C172 ON LANDING ROLLOUT RUNWAY 28 OPPOSITE DIRECTION. THE PILOT OF LANDING C172 LATER STATED THEY OBSERVED THE DEPARTING C172 LESS THAN 500 FEET IN FRONT OF THEM AND APPROXIMATELY 20 FEET OFF THE GROUND. ALSO, THE LANDING C172 REPORTED BEING AT TAXI SPEED WHEN THE DEPARTURE FLEW OVER AT AN ESTIMATED 30-40 FEET VERTICAL, TOOK NO EVASIVE ACTIONS, AND THEN EXITED THE RUNWAY AT C4. ATCT ESTIMATED THE DEPARTING C172 ROTATED 500 FEET FROM THE C172 ON ROLLOUT FOR RUNWAY 28.

A FALCON F900 WAS ISSUED TAXI INSTRUCTIONS VIA TAXIWAY FOXTROT, ALPHA-ALPHA, HOLD SHORT OF ECHO ON ALPHA-ALPHA. PILOT READ BACK WAS CORRECT. ONE MINUTE LATER GROUND SEQUENCE (GC-SEQ) INSTRUCTED THE F900 TO CONTINUE VIA ALPHA-ALPHA, ECHO, DELTA-DELTA, WHICH PILOT READ BACK. THE F900 FAILED TO MAKE THE LEFT TURN ONTO ALPHA-ALPHA AND CROSSED THE HOLD SHORT LINE ON DELTA FOR RUNWAY 4 THUS CONFLICTING WITH A DEHAVILLAND DH8A TOUCHING DOWN ON RUNWAY 4. GC-SEQ IMMEDIATELY INSTRUCTED THE F900 TO HOLD SHORT OF RUNWAY 4 AND PILOT RESPONDED HOLDING SHORT. THE F900 STOPPED JUST PRIOR TO RUNWAY EDGE AS THE DH8A PASSED THE F900’S POSITION ON DELTA. WING SPAN OF DH8A IS 85 FEET WITH RUNWAY WIDTH OF 150 FEET, THE CLOSEST HORIZONTAL PROXIMITY BETWEEN AIRCRAFT IS ESTIMATED AT BETWEEN 35-50 FEET. THIS EVENT MET THE PARAMETERS ESTABLISHED WITHIN ASDE-X TO CAUSE AN ALARM AND AN ALERT WAS RECEIVED. AIRPORT OPERATIONS INDICATE ELEVATED RUNWAY GUARD LIGHTS (ERGL) AND RUNWAY 4-22 MANDATORY SIGNS AT DELTA IN SERVICE AT THE TIME OF EVENT. MOST OF THE IN- PAVEMENT RGL'S ON WEST SIDE OF RUNWAY 4-22 ARE NOTAMED OUT OF SERVICE DUE TO CONSTRUCTION. ADDITIONALLY, THE AREA HAS ENHANCED TAXIWAY CENTERLINE MARKINGS AND SURFACE PAINTED HOLDING POSITION SIGNS.

THE BE55 THEN ENTERED RUNWAY 14 AT C8 WITHOUT CLEARANCE THUS CONFLICTING WITH A AERO-COMMANDER AC11 OVER THE LANDING THRESHOLD FOR SAME RUNWAY AT APPROXIMATELY 50 FEET VERTICAL. GC AND LC INSTRUCTED THE BE55 TO STOP. THE AC11 ADDED POWER AND MANEUVERED SLIGHTLY LEFT TO NOT FLY OVER THE BE55 AND THEN LANDED. THE BE55 STOPPED ON THE RUNWAY WITH AIRCRAFT TAIL SLIGHTLY OVER THE RUNWAY EDGE. BE55 LENGTH IS 28 FEET, RUNWAY WIDTH IS 100 AND CLOSEST HORIZONTAL IS ESTIMATED AT 25 FEET AND THE AC11 WAS ESTIMATED TO BE BETWEEN 25-50 FEET VERTICAL AS IT PASSED THE BE55. HORIZONTAL DISTANCE FROM APPROACH END TO C8 IS APPROXIMATELY 750 FEET.

TRAINING IN PROGRESS ON NORTH LOCAL (NLC) POSITION. A EMBRAER E145 WAS CLEARED FOR TAKEOFF ON RUNWAY 32L ABEAM TAXIWAY T10 BY LOCAL 3 (LC3) WITH A EMBRAER E135 ON AN APPROXIMATELY 3.5 MILE FINAL FOR RUNWAY 9R. THE FLIGHT PATHS FOR RUNWAY 32L AND 9R INTERSECT. LC3 DID NOT SEE OR WAS AWARE OF THE RUNWAY 9R ARRIVAL. UPON RECOGNIZING THE CONFLICT, LC3 ISSUED THE DEPARTING E145 THE CROSSING E135 TRAFFIC AND INSTRUCTED THE PILOT TO STAY AS LOW AS YOU CAN. E145 PILOT STATED "YEAH, WERE DOING THAT". LC3 THEN INSTRUCTED NLC TO SEND THE E135 AROUND. THE E135 CROSSED OVER RUNWAY 32L BY TAXIWAY T2 AND STARTED A CLIMB WHEN THE E145 WAS JUST SOUTH OF T2 AND BECOMING AIRBORNE. CLOSEST PROXIMITY ESTIMATED AT THAT POINT WAS 125 FEET VERTICAL, 350 FEET LATERAL. THIS EVENT DID NOT MEET THE PARAMETERS ESTABLISHED WITHIN ASDE-X (NON-INTERSECTING RUNWAYS) TO CAUSE AN ALARM AND NO ALERT WAS RECEIVED. E145 PILOT STATEMENT: DEPARTING 32L AT T10 AT CHICAGO O'HARE WE BEGAN THE TAKEOFF ROLL NORMALLY. I CALLED 80 KNOTS AND CONTINUED TO SCAN INSIDE AND OUT. AT ABOUT 115 KNOTS I LOOK DOWN TO CHECK FOR V1 SPEED AND NOTICE AN AMBER TARGET 200 ABOVE US ON THE TCAS. I MOVED FORWARD TO LOOK AROUND THE WINDOW POST AND NOTICE AN EAGLE E145 ON SHORT FINAL FOR 9R. I QUICKLY PUT MY HAND ON THE YOKE TO DELAY THE ROTATION AND POINTED OUT THE TARGET TO MY FO. HE ASKED IF WE SHOULD ABORT BUT WE WERE PAST V1 AND RUNNING OUT OF PAVEMENT, I SAID NO JUST KEEP ROLLING AND ROTATE PAST HIS ARRIVAL PATH. ATCT THEN GAVE US A TRAFFIC ALERT AND TOLD US TO STAY LOW, THAT ADVICE MAY HAVE BEEN TO LATE IF WE DIDN'T SEE HIM FIRST. HE MAY HAVE CLEARED THE AIRCRAFT BY 150 FT AT THE POINT HE CROSSED OVER. WE CONTINUED OUR CLIMB AND THE FLIGHT WITHOUT INCIDENT.

A CIRRUS SR22 CALLED INBOUND FROM THE WEST AND WAS TOLD TO REPORT 5 MILES SOUTHWEST. A SECOND SR22 WAS ALSO INBOUND TO ATW. A EMBRAER E135 CHECKS IN ON FINAL AND IS CLEARED TO LAND ON INTERSECTING RUNWAY 30. LC ASKED THE FIRST SR22 TO SAY POSITION AND PILOT REPLIED THEY WERE JUST SOUTH OF THE AIRPORT. LC ASKED IF THE WERE AT 2,900 FEET AND PILOT STATED THEY WERE. LC TOLD THE FIRST SR22 TO DISREGARD THE POSITION REPORT AND THEN CLEARED THE AIRCRAFT TO LAND ON RUNWAY 3. LOCAL (LC) ASKED WHO THE AIRCRAFT 2 MILES SOUTH OF THE FIELD WAS. THE SECOND SR22 STATED THAT IT WAS HIM WHEN IN ACTUALITY IT WAS THE FIRST SR22. BASING SEPARATION ON THE SECOND SR22’S POSITION REPORT, LC CLEARED THE SECOND SR22 TO LAND RUNWAY 3 AND TO HOLD SHORT OF RUNWAY 30. LAND AND HOLD SHORT OPERATIONS (LAHSO) INVOLVING AIR CARRIERS, IS NOT AUTHORIZED AT ATW. THE SECOND SR22 READ BACK THE CLEARANCE AND HOLD SHORT INSTRUCTION. THE FIRST SR22 THEN LANDED ON RUNWAY 3 PRIOR TO THE SECOND SR22 AND DID NOT HOLD SHORT OF RUNWAY 30 THUS CONFLICTING WITH THE E135 LANDING ON INTERSECTING RUNWAY 30. WHEN THE E135 PILOT REALIZED THE SR22 WAS NOT GOING TO HOLD SHORT OF RUNWAY 30, THEY APPLIED MAXIMUM REVERSE AND MAXIMUM BRAKING, STOPPING APPROXIMATELY 20 FEET FROM THE EDGE OF INTERSECTING RUNWAY 3/30. THE FIRST SR22 CONTINUED TAXI THROUGH THE INTERSECTION AND EXITED AT ALPHA. THE WIDTH OF RUNWAY 30 IS 150 FEET, CENTERLINE TO RUNWAY EDGE IS 75 FEET, WITH CLOSEST PROXIMITY ESTIMATED AT 100 FEET. DISTANCE FROM RUNWAY 30 APPROACH END TO INTERSECTION OF 30/3 IS APPROXIMATELY 3,600 FEET.
