THE PILOT INFORMED THE TOWER THAT A PEDESTRIAN IS WALKING IN THE MIDDLE OF RUNWAY 01 TOWARDS THE APPROACH END. THE PERSON WAS APPROXIMATELY 200 FEET SOUTH OF THE RUNWAY 32/14 HOLD SHORT LINES. THE CLOSEST PROXIMITY WAS 150 FEET AGL VERTICAL, AND 0 LATERAL.

    A/C 1/H25A WAS TAXING WESTBOUND ON TAXIWAY NOVEMBER AND WAS TOLD TO HOLD SHORT OF RUNWAY 18 AND TO REPORT HOLDING. A/C 2/CRJ2 WAS HOLDING AT THE ILS CRITICAL HOLD AREA ON RUNWAY 18 AT TAXIWAY ALPHA AND WAS CLEARED FOR TAKEOFF. APPROXIMATELY 85 SECONDS AFTER THE TAKEOFF CLEARANCE WAS ISSUED, H25A ADVISED LOCAL CONTROL (LC) THAT HE WAS HOLDING SHORT OF RUNWAY 18 AT TAXIWAY NOVEMBER. LC CLEARED H25A TO CROSS RUNWAY 18 AND TURN RIGHT ON TAXIWAY ALPHA. APPROXIMATELY 35 SECONDS LATER, H25A ADVISED LC THEY HAD STOPPED WITH THEIR NOSEWHEEL ACROSS THE RUNWAY EDGE LINE AND THAT AN AIRCRAFT PASSED OVER THEM. AFTER FURTHER DISCUSSION WITH H25A, IT WAS DETERMINED THAT HE STOPPED THE AIRCRAFT AS CRJ9 PASSED IN FRONT OF THEM AT AN ALTITUDE OF APPROXIMATELY 30 FEET. THE ESTIMATED CLOSEST PROXIMITY IS 32 FEET LATERAL AND 30 FEET VERTICAL. POOR VISIBILITY PREVENTED LC FROM SEEING THE SURFACE MOVEMENT AREA (18 RVR EQUALLED 2600 FEET).

AS A/C 2 WAS APPROXIMATELY 1.5 NM FINAL, A/C 1 ANNOUNCED ON THE APPROACH CONTROL FREQUENCY SHE WAS DEPARTING RUNWAY 23. A/C 2 QUESTIONED ATC ABOUT THE TRAFFIC ON THE RUNWAY AND LOCAL CONTROL OBSERVED A/C 1 ON DEPARTURE ROLL WITHOUT AUTHORIZATION. AT APPROXIMATELY 1/3 NM FINAL LOCAL CONTROL ISSUED GO-AROUND INSTRUCTIONS TO A/C 2. A/C 2 FLEW OVER A/C 1 AND THE CLOSEST PROXIMITY WAS APPROXIMATELY 250 FEET VERTICAL.

    A/C 1/HXA CALLED READY FOR TAXI FROM THE HANGARS ON THE WEST SIDE OF THE AIRPORT. GROUND CONTROL (GC) ISSUED INSTRUCTIONS FOR HXA TO TAXI TO RUNWAY 3 VIA TAXIWAY LIMA 3, TURN RIGHT ON TAXIWAY LIMA THEN VIA TAXIWAYS DELTA AND FOXTROT. READ BACK WAS CORRECT. HXA MADE A LEFT TURN ON TAXIWAY DELTA AND CROSSED RUNWAY 1 WITHOUT AUTHORIZATION. A/C 2/C210 WAS LANDING ROLL ON RUNWAY 1 AND ADVISED LOCAL CONTROL THAT AN AIRCRAFT HAD CROSSED RIGHT IN FRONT OF HIM. TOWER PERSONNEL THEN OBSERVED HXA HAD CROSSED THE RUNWAY. GC INSTRUCTED HXA TO HOLD POSITION. DUE TO THE SPEED OF C210, NO EVASIVE MANEUVER WAS PERFORMED. THE CLOSEST ESTIMATED PROXIMITY WAS APPROXIMATELY 50 FEET. THIS EVENT OCCURRED IN HOT SPOT 3.

E145 CONTINUED AND TURNED SOUTH ON TAXIWAY TANGO. A SECOND E145 WAS NOT SENT AROUND AND OVERFLEW THE TAXIING AIRCRAFT WHILE LANDING RUNWAY 9R. THIS OCCURRED AT HOT SPOT 6. CLOSEST PROXIMITY ACCORDING TO ASDE ANALYSIS WAS 14 FEET LATERAL AND 76 FEET VERTICAL.

ASDE-X ANALYSIS INDICATED THAT THE B744 BANKED SLIGHTLY TO THE LEFT AND THE VEHICLE DRIVER WAS PULLING OFF THE RUNWAY BETWEEN TAXIWAYS HOTEL 2 AND HOTEL. CLOSEST PROXIMITY WAS DETERMINED TO BE 95 FEET VERTICAL AND LESS THAN 100 FEET LATERAL.

    MD520 (HELO) WAS LOCATED ON THE SOUTH RAMP IN BETWEEN TAXIWAYS ECHO AND PAPA AND CALLED LOCAL CONTROL (LC) FOR A WEST LOCH DEPARTURE (WLD) AND REQUESTED A TURN TO THE EAST WHEN ABLE. WLD IS A VFR DEPARTURE PROCEDURE THAT ROUTES AIRCRAFT TO THE NORTHWEST OF HNL BEFORE TURNING ON COURSE. LC CONTROLLER CLEARED THE MD520 TO DEPART DIRECTLY TO THE EAST WITH NO DEPARTURE RESTRICTIONS. MD520 DID NOT HEAR THE FULL CLEARANCE AND REQUESTED THE LC CONTROLLER TO REPEAT. LC CONTROLLER THEN CLEARED THE MD520 DEPARTURE DIRECT TO PALI. MD520 DEPARTED THE SOUTH RAMP NORTHWEST BOUND INTO THE WIND BEFORE TURNING DIRECT PALI. MD520 OVERFLEW PA23 ON LANDING ROLL ON RUNWAY 22L BY AND ESTIMATED 95 FEET.
