    A/C 1 /A319 WAS ON DEPARTURE ROLL RY 16 RIGHT, REPORTED TO TOWER THAT HE HAD ALMOST HIT SOMEONE. TOWER COULD NOT INITIALLY SEE ANYONE BUT THEN OBSERVED A PERSON WHO APPEARED TO BE WALKING ALONG THE EDGE OF RUNWAY 16 RIGHT NEAR TAXIWAY JULIET. TOWER ADVISED AIRPORT MAINTENANCE OF THE INDIVIDUAL?S LOCATION AND MAINTENANCE, WHO WAS IN THE VICINITY, ESCORTED THE PERSON FROM THE RUNWAY. AIRPORT POLICE AND OPERATIONS WAS NOTIFIED AND PERSON APPREHENDED AND TAKEN INTO CUSTODY BY AIRPORT POLICE. INTERVIEW WITH OPERATIONS PERSONNEL AND LAW ENFORCEMENT INDICATES THE INDIVIDUAL WAS ONLY PARTIALLY COHERENT. HE REPORTED TO AUTHORITIES THAT SOMEONE OFF AIRPORT WAS CHASING HIM AND THE ONLY ESCAPE ROUTE WAS ACROSS THE AIRPORT. POLICE REPORTS THIS INDIVIDUAL HAS A HISTORY OF TRESPASSING. CLOSEST PROXIMITY WAS 15 FEET.

    A/C 1/BE20 WAS INSTRUCTED TO CROSS RUNWAY 11R, HOLD SHORT OF RUNWAY 11L, ON TAXIWAY DELTA. READ BACK WAS CORRECT. RUNWAY 11L WAS UNDER CONSTRUCTION AND SHORTENED RUNWAY OPERATIONS WERE IN EFFECT. RUNWAY 11L THRESHOLD WAS RELOCATED NORTH OF RUNWAY 3/21 NEAR TAXIWAY ALPHA 1 TO SUPPORT THE SHORTEN RUNWAY OPERATIONS. FLIGHT OF 4 F16S WERE LANDING RUNWAY 11L. F16-1 HAD LANDED AND WAS ISSUED EXIT INSTRUCTIONS. BE20 TAXIED ONTO RUNWAY 11L AT TAXIWAY DELTA NORTHEAST BOUND WITHOUT AUTHORIZATION. LC SENT THE REMAINING FLIGHT AROUND. F16-2 WAS SENT AROUND PRIOR TO TAXIWAY DELTA DUE TO BE20 ENTERING THE RUNWAY. F16-3 WAS SENT AROUND ON A HALF-MILE FINAL, F16-4 WAS SENT AROUND ON A 1.4 MILE FINAL. ACCORDING TO CHIEF OF SAFETY, 162ND WING, AZ ANG, F16-2 WAS INSTRUCTED TO GO AROUND AT APPROXIMATELY 10 FEET ABOVE THE SURFACE. THE PILOT APPLIED FULL MILITARY POWER, MOMENTARILY TOUCHED DOWN AND THEN WAS AIRBORNE JUST PRIOR TO TAXIWAY DELTA. WING CHIEF OF SAFETY REVIEW OF THE F16-2 HUD SHOWS THE F16 PASSED ABEAM TAXIWAY DELTA ABOVE AND ALONG THE LEFT EDGE OF RUNWAY 11L AT 20 FEET AGL (RADAR ALTITUDE). THE PILOT OF THE F16 SEEN THE BE20 AS HE WENT BY HIS RIGHT WING, WITH THE NOSE OF THE BE20 AT THE RUNWAY CENTERLINE. CLOSEST HORIZONTAL DISTANCE IS ESTIMATED TO BE 58 FEET BASED ON THE F16 WING SPAN OF 32? 9? AND THE 150? WIDE RUNWAY. CLOSEST PROXIMITY 20? VERTICAL, ESTIMATED 58? FEET HORIZONTAL. WAS UNABLE TO DETERMINE WHERE EXACTLY F16-2 TOUCHED DOWN.

    A/C 1 C172 CROSSED THE HOLD LINE AND ENTERED RWY 7R WITHOUT AUTHORIZATION OR CLEARANCE CONFLICTING WITH A/C 2 LESS THAN ONE-MILE FINAL. A/C 1 CALLED READY FOR DEPARTURE RUNWAY 7R WHEN A/C 2 C172 WAS LESS THAN ONE-MILE FINAL AND WAS CLEARED FOR THE OPTION RUNWAY 7R. ATC INSTRUCTED A/C 1 TO HOLD SHORT OF RUNWAY 7R AND HE READ IT BACK CORRECTLY. ATC NOTICED THAT A/C 2 WAS EXECUTING A GO-AROUND AND THEN OBSERVED A/C 1 ENTERING RUNWAY 7R WITHOUT ATC AUTHORIZATION. ALL INDICATIONS ARE THAT A/C 2 WAS ABOUT 53 FEET ABOVE THE GROUND AND OFF SET TO THE LEFT OF A/C 1 AND DID NOT DIRECTLY OVERFLY THE A/C 2 AND THAT A/C 1 ENTERED THE RUNWAY BEHIND A/C 2 AS A/C 2 WAS CLIMBING OUT.

A/C 2/C172 LANDED RUNWAY 30L AND OBSERVED A/C 1 ON FINAL OPPOSITE DIRECTION. A/C 2 PULLED OFF TO THE RIGHT SIDE OF THE RUNWAY. A/C 1 INITIATED A GO-AROUND AND APPEARED TO OVERFLY A/C 2. LOSS OF SEPARATION, CLOSEST PROXIMITY WAS 100 FEET VERTICAL.

    BOTTOM LINE: A/C 1/EC45, AFTER LANDING ON RUNWAY 30 WITH A PROPER CLEARANCE AND HOLDING AT THE NUMBERS, TOOK EVASIVE ACTION TO EXIT THE RUNWAY WHILE A/C 2/A306 WAS LANDING RUNWAY 30. CLOSEST PROXIMITY 100 FEET HORIZONTAL AND LESS THAN 200 FEET VERTICAL. SUMMARY: A/C 1/EC45 WAS ON THE ILS APPROACH TO RUNWAY 30 AND STATED THAT THEY WILL BE DELAYING ON THE GROUND, AND REQUESTED A 'SIDE STEP' TO PICK UP AN IFR TEC ROUTE TO BUR. LOCAL CONTROL (LC) INSTRUCTED THE EC45 TO LAND ON THE NUMBERS OF RUNWAY 30. THE PILOT READ BACK THE CLEARANCE CORRECTLY. ON APPROXIMATELY 2 MILE FINAL THE EC45 AGAIN ADVISED TOWER THEY WOULD BE ON THE GROUND ABOUT TEN MINUTES TO PICK UP THEIR IFR DEPARTURE CLEARANCE AND LC RESTATED THEY WERE CLEARED TO LAND ON THE NUMBERS OF RUNWAY 30. APPROXIMATELY ONE MINUTE AFTER THE EC45 ENTERED OVER THE RUNWAY 30 DISPLACED THRESHOLD, LC CLEARED A/C 2/A306 TO LAND RUNWAY 30. LC DID NOT ISSUE RUNWAY EXITING INSTRUCTIONS TO THE EC45. APPROXIMATELY TWO MINUTES LATER, THE A306 WAS OVER THE RUNWAY 30 DISPLACED THRESHOLD. APPROXIMATELY 15 SECONDS LATER, AFTER EXITING RUNWAY 30 AT TAXIWAY D3 OF THEIR OWN ACCORD, THE EC45 ASKED LC TO 'MARK TAPE'. ACCORDING TO POST-EVENT PILOT INTERVIEWS, THE EC45, WHILE HOLDING ON RUNWAY 30, ROTATED THE HELICOPTER TO THE LEFT, LOOKED UP THE FINAL APPROACH AND OBSERVED THE A306. THE FIRST OFFICER SHOUTED 'MOVE MOVE' AND THE PILOT WAS ABLE TO TAKE QUICK ACTION AND CLEAR THE RUNWAY. THE A306 CAPTAIN STATED THAT AT ABOUT 1500 FEET, THEN AGAIN AT 500 FEET, THEY SAW WHAT LOOKED LIKE A BLACK SPOT ON THE APPROACH END OF THE RUNWAY BUT WEREN'T SURE WHAT IT WAS, AND GUESSED THAT IT WAS A RUBBER SPOT. AS THEY CROSSED THE THRESHOLD THEY OBSERVED A HELICOPTER QUICKLY EXITING THE RUNWAY. CLOSEST PROXIMITY APPROXIMATELY 100 FEET HORIZONTAL AND LESS THAN 200 FEET VERTICAL. NOT A HOT SPOT EVENT.

GROUND CONTROL ALERTED LC THAT A/C 2 WAS CROSSING RUNWAY 23L AT TAXIWAY C WITH A/C 1 ON THE ROLL. LC TRIED TO STOP A/C 1, BUT THE AIRCRAFT WAS UNABLE TO COMPLY. A/C 2 NOTICED THE CONFLICT WITH THE DEPARTING TRAFFIC AND ADVISED LC THAT THE HE WOULD HOLD POSITION. A/C 2 STATED HE HAD TO BRAKE HARD AND STOPPED WITH THE NOSE OF THE AIRCRAFT SHORT OF THE RUNWAY EDGE LINE. A/C 1 WAS NOT YET AIRBORNE WHEN PASSING A/C 2 HOLDING ON TAXIWAY C AT 23L. CLOSEST PROXIMITY APPROXIMATELY 75 FEET HORIZONTAL.

PER ASSC ANALYSIS, A/C 1 BEGAN UPWARD TREND IN ATTITUDE, BASED ON AIRCRAFT?S TRANSPONDER, 950 FEET HORIZONTALLY FROM A/C 2. A/C 1 TRANSPONDER UPDATES INDICATES 125 FEET AGL WHEN IT OVER FLIES A/C 2. CLOSEST VERTICAL PROXIMITY ESTIMATED AT 87 FEET. A/C 1 FLIGHT CREW UNAWARE OF AIRCRAFT THAT WAS LINED UP ON RUNWAY 28L. NOTE THE POSITIONAL ACCURACY OF THE ASSC IS +/- 20 FT.
