LC INSTRUCTED A/C 1 TO CONTINUE INBOUND AND REPORT CIRCLING SOUTHBOUND FOR RUNWAY 10R. READ BACK CORRECT FROM A/C 1. A/C 1 STATES "I'LL TAKE 10L IF THAT'S OKAY?" LC ADVISES A/C 1 THAT HE HAS THEIR REQUEST. A/C 2 R22 WAS OPERATING IN THE PRACTICE VFR PATTERN ON RWY 10L. LC THEN ADVISED A/C 1 OF A/C 2 VIA A TRAFFIC CALL. A/C 1 REPORTED LOOKING FOR TRAFFIC. A/C 1 REPORTED THEIR CIRCLING MANEUVER. LC ADVISED A/C 2 OF A/C 1 VIA A TRAFFIC CALL. A/C 2 REPORTED LOOKING FOR A/C 1. A/C 1 WAS ONCE AGAIN ADVISED OF A/C 2 ON THE PARALLEL RUNWAY AND CLEARED TO LAND RUNWAY 10R. GOOD READ BACK FROM A/C 1. LC THEN COORDINATED WITH SCT TO ADVISE THEM THAT A/C 1 WOULD BE CIRCLING FOR RUNWAY 10R. A/C 2 WAS OPERATING OVER THE NUMBERS OF RUNWAY 10L AS A/C 1 ROLLED OUT ON FINAL AND LINED UP FOR THE WRONG RUNWAY, RUNWAY 10L. THE LC DETECTED THE EVENT AND ISSUED IMMEDIATE GO AROUND INSTRUCTIONS. A/C 1 REPORTED TAKING EVASIVE ACTION TO MISS A/C 2. THE CLOSEST PROXIMITY REPORTED WAS 50 TO 100 FEET LATERALLY AT THE SAME ALTITUDE. CONFLICT DETERMINED AS A/C 1 LINED UP FOR THE WRONG RUNWAY AND EXECUTED EVASIVE ACTION TO AVOID A/C 2 ON THE PARALLEL RUNWAY. NO SAFETY ALERTS WERE ISSUED.

CLOSEST PROXIMITY BETWEEN THE TWO AIRCRAFT WAS 61FT IN ELEVATION AND 630FT LATERAL.

LC CLEARED C152 FOR TAKEOFF RUNWAY 29R WITH NO DELAY ON THE ROLL. C152 TAXIED ONTO RUNWAY 29R AND BEGAN ITS TAKEOFF ROLL. RUNWAY SEPARATION WAS LOST WHEN R22 OVER FLEW THE C152 WITH A CLOSEST VERTICAL DISTANCE OF 100 FEET. C152 ABORTED THEIR TAKEOFF AND EXITED RUNWAY 29R AT TAXIWAY CHARLIE.

WHEN A/C 2 WAS APPROACHING RWY 32 THRESHOLD, A/C 1 WAS CLEARED FOR TAKEOFF AND ISSUED A TRAFFIC ADVISORY REFERENCING AN AIRCRAFT ON FIVE MILE FINAL BUT DID NOT REFERENCE A/C 2. A/C 2 WAS ON LANDING ROLL OUT WHEN A/C 1 TAXIED ONTO RWY 32. THE LC STATED STOP STOP STOP. NO OTHER INSTRUCTIONS WERE GIVEN. A/C 1 MADE A 180 DEGREE TURN AND EXITED THE RUNWAY WHILE A/C 2 VEERED TO AVOID A/C 1. THE LC STATED THAT HE FORGOT ABOUT A/C 2 WHEN CLEARING A/C 1 FOR TAKEOFF. CLOSEST PROXIMITY ESTIMATED BY THE FACILITY WAS 25 FEET. VALIDATED LOSS OF SEPARATION. NO PILOT DEVIATION.

A/C 1 BE35 ATTEMPTED TO LAND ON A CLOSED RUNWAY. A PREVIOUS AIRCRAFT ACCIDENT HAD OCCURRED ON RWY25R AND AIRPORT MANAGEMENT TEMPORARILY CLOSED THE AIRPORT. ATC HAD INCORRECTLY RECORDED AN NEW ATIS BROADCAST AFTER THAT EVENT, WHICH INDICATED THE ATCT WAS CLOSE. IT IS UNKNOWN IF A/C 1 BE35 HAD RECEIVED THAT INFORMATION. A/C 1 BE35 CALLED IN ON HWD LC FREQUENCY AND ASKED IF ANYONE WAS USING THE FREQUENCY. HWD ATCT REPLIED AND RECEIVED NO FURTHER REPLY. ATC NOTICE AN AIRCRAFT ON SHORT FINAL TO RWY 25R AND ISSUED A GO-AROUND IN THE BLIND. NO REPLY WAS RECEIVED, BUT A/C 1 BE35 WAS OBSERVED GOING AROUND AND OVER FLEW THE MEN AND EQUIPMENT ON RWY 25R BY 20 FEET, AS REPORTED BY THE ATCT. AFTER DEPARTING THE AREA, THE AIRCRAFT WAS TRACKED TO A LANDING AT KSJC. LOSS OF RUNWAY SEPARATION OCCURRED.

A/C 1 CALLED FOR TAXI AND GROUND CONTROL (GC) ISSUED INSTRUCTIONS FOR THE E145 TO TAXI TO RUNWAY 20. A/C 1 REQUESTED RUNWAY 14. GC ISSUED INSTRUCTED A/C 1 TO TAXI VIA TAXIWAYS FOXTROT AND WHISKEY TO RUNWAY 14 AND ADVISED THE E145 THAT THEY HAD A RELEASE TIME OF 1751Z. LOCAL CONTROL (LC) INSTRUCTED A/C 1 TO LINE UP AND WAIT ON RUNWAY 14. VEH 1 CALLED GC AT THE INTERSECTION OF TAXIWAYS NOVEMBER AND CHARLIE AND REQUESTED TO PROCEED TO AN AREA ON THE SOUTH WEST SIDE OF THE AIRFIELD. GC INSTRUCTED VEH 1 TO CROSS RUNWAY 20 AT TAXIWAY CHARLIE AND PROCEED VIA TAXIWAYS UNIFORM AND ECHO. LC CLEARED A/C 1 FOR TAKEOFF RUNWAY 14. GC LOOKED AND DID NOT SEE THE MEMORY AID SHOWING RUNWAY 14 HOT AND INSTRUCTED VEH 1 TO CROSS RUNWAY 14 AT TAXIWAY UNIFORM. GC HAD FORGOTTEN THAT THE E145 HAD BEEN TAXIED TO RUNWAY 14 TO WAIT FOR RELEASE. A/C 1 WAS ROLLING ON RUNWAY 14 WHEN THE VEHICLE CROSSED THE RUNWAY. THE PILOT STATED TRUCK TRUCK TRUCK OVER THE FREQUENCY. THE E145 BEGAN TO LIFT OFF JUST PRIOR TO TAXIWAY UNIFORM. THE CLOSEST ESTIMATED PROXIMITY WAS 500 FEET LATERAL REPORTED BY THE PILOT OF A/C 1 .

SUMMARY : HELICOPTER (A/C 1) WAS DOING PATTERN WORK ON TAXIWAY CHARLIE AND A PA44 (A/C 2) ON AN ILS PRACTICE APPROACH FOR RUNWAY 24 WAS DOING A MISSED APPROACH. THE HELICOPTER WAS CLEARED FOR THE OPTION ON TAXIWAY CHARLIE AND WAS NOT GIVEN RESTRICTIONS TO REMAIN SOUTHEAST OF RUNWAY 24. THE ONLY RESTRICTIONS THAT WERE GIVEN TO THE HELICOPTER WERE TO REMAIN EAST OF RUNWAY 33 AT ALL TIMES BY THE PREVIOUS CONTROLLER. THE CURRENT CONTROLLER INCORRECTLY ASSUMED THE HELICOPTER WAS RESTRICTED ON ALL RUNWAYS AFTER BRIEFING. N6PC WAS CLEARED FOR THE OPTION ON RUNWAY 24 AND N4SH WAS SITTING ON TAXIWAY CHARLIE WHEN A PHONE CALL WAS RECEIVED ON THE TOWER PHONE. IT WAS ANOTHER CONTROLLER WHO SAID GATE 17 WASN'T CLOSING BEHIND THEM. THE LOCAL CONTROLLER CALLED THE SUPERVISOR WHO TOLD THE CONTROLLER TO ADVISE GROUND CONTROL TO SEND AIRPORT SECURITY TO THE GATE 17. THERE WAS TRAINING ON GROUND CONTROL AND THEY WERE ADVISED TO DO SO. LOCAL CONTROL THEN SCANNED AND COULD SEE THAT N4SH WAS NORTHEAST OF RUNWAY 24 AND N6PC WAS ON THE UPWIND RUNWAY 24. N6PC ADVISED THE TOWER THAT THE HELICOPTER HAD CUT IN FRONT OF HIM, IT WAS VERY CLOSE AND THEY HAD TO MAKE AN EVASIVE MANEUVER IN ORDER TO AVOID THE HELICOPTER. QA SUMMARY : REVIEWED FALCON REPLAY, AUDIO, WEATHER, AND SEPARATION DETAILS. A/C 1 WAS CONDUCTING OPTION APPROACHES TO TAXIWAY CHARLIE (EAST AND PARALLEL TO RUNWAY 33). A POSITION RELIEF BRIEFING HAD BEEN COMPLETED AND THE RELIEVING CONTROLLER BELIEVED A/C 1 WAS RESTRICTED TO REMAIN EAST OF RUNWAY 33IN RIGHT HAND PATTERN. A/C 2 WAS MAKING A VFR APPROACH TO RUNWAY 24 AND CLEARED FOR THE OPTION. A/C 1 WAS ALSO CLEARED FOR THE OPTION TAXIWAY CHARLIE STILL ONLY RESTRICTED EAST OF RUNWAY 33. A/C 1 PASSED JUST IN FRONT OF A/C 2 OVER RUNWAY 24, LANDED AND COME TO A STOP NORTHEAST OF RUNWAY 24 ON TAXIWAY CHARLIE. THE PILOT OF A/C 2 STATED VIA TELEPHONE THAT THE HELICOPTER HAD BEEN OVER RUNWAY 24 AND THAT THE CLOSEST PROXIMITY ESTIMATED LESS THAN 100 FEET. VALIDATED LOSS. RIS FINDINGS : NA THE ORIGINAL FACILITY WRITE UP STATES THAT THE PILOT OF A/C 2 PA44 REPORTED TO THE TOWER THAT A/C 1 (HELO) ?CUT IN FRONT OF HIM, IT WAS VERY CLOSE AND THAT HE (A/C 2) MADE AN EVASIVE MANEUVER TO AVOID THE HELICOPTER AND THAT THE CLOSEST DISTANCE WAS LESS THAN 100FT

LOCAL CONTROL THEN CLEARED A/C 1 FOR TAKEOFF WHILE THE VEHICLE WAS SITTING OFF THE EAST EDGE OF RY 22 AT TAXIWAY CHARLIE 1. THE FACILITY ESTIMATED THE CLOSEST PROXIMITY AS 0 FEET VERTICAL, AND 75 FEET LATERAL.
