A PEDESTRIAN ENTERED RWY 17L WITHOUT ATC AUTHORIZATION. A PEDESTRIAN BREACHED THE PERIMETER FENCE AND ENTERED RWY 17L FROM THE EAST SIDE NEAR TWY P7. AIRCRAFT 2/A21N WAS DEPARTURE ROLL RWY 17L AND STRUCK THE PEDESTRIAN APPROXIMATELY 3000 FEET FROM THE APPROACH END OF THE RUNWAY RESULTING IN A FATALITY. AIRCRAFT 2 EXPERIENCED AN ENGINE FIRE AND ABORTED TAKEOFF. EMERGENCY RESPONSE ENSUED AND THE PASSENGERS AND CREW EVACUATED THE AIRCRAFT ON THE RUNWAY.

DRIVER READ BACK, TURN ON TWY F AND THEN TWY B, Z, Z2. VEHICLE MISSED THE TURN ON TWY F AND GC INSTRUCTED THE DRIVER TO STOP. VEHICLE STOPPED LESS THAN 80 FT FROM THE RWY 28L RUNWAY EDGE LINE. AIRCRAFT 2/B772 WAS DEPARTING RWY 28L AND PASSED VEHICLE WITH THE LEFT WINGTIP SLIGHTLY OVER THE RWY 28L RUNWAY EDGE LINE. LESS THAN 80 FT EXISTED BETWEEN AIRCRAFT 2 AND THE FRONT OF THE TUG.

AIRCRAFT 1 READ BACK "D, CROSS 17R, NO DELAY". LC DID NOT CORRECT THE BAD READ BACK. AIRCRAFT 1 CROSSED RWY 17R AT TWY D. AIRCRAFT 2/DA40 WAS DEPARTING RWY 17R AND OVERFLEW AIRCRAFT 1 BY AN ESTIMATED 100 FEET.

LC INSTRUCTED VEHICLE TO CROSS RWY 4 AT TWY D. AIRCRAFT 2/CRJ9 WAS CLEARED TO LAND RWY 4 AND WAS APPROXIMATELY 900 FEET FROM THE LANDING THRESHOLD. LC REALIZED THE CONFLICT AND INSTRUCTED VEHICLE TO STOP AS AIRCRAFT 2 WAS ABEAM TWY Y ON LANDING ROLLOUT, BUT NO RESPONSE WAS RECEIVED. LC AGAIN INSTRUCTED VEHICLE TO STOP AS AIRCRAFT 2 WAS NOW ABEAM TWY C, BUT NO RESPONSE RECEIVED. VEHICLE AND AIRCRAFT 2 COLLIDED AT TWY D.

ATC DID NOT PROVIDE RUNWAY SEPARATION BETWEEN VEHICLE AND AIRCRAFT 2. LC CLEARED AIRCRAFT 2/CRJ9 FOR TAKEOFF RWY 13R. LC THEN INSTRUCTED VEHICLE/MAINTENANCE FLIGHT TO CROSS RWY 13L AND RWY 13R ON TWY K. VEHICLE CROSSED RWY 13L AND ENTERED RWY 13R AT TWY K AS AIRCRAFT 2 WAS PASSING TWY H. THE ASDE ALERTED AND LC INSTRUCTED VEHICLE TO HOLD SHORT OF RWY 13R, BUT THE AIRCRAFT WAS ALREADY IN THE MIDDLE OF THE RUNWAY. VEHICLE CLEARED RWY 13R AT TWY K PRIOR TO AIRCRAFT 2 REACHING THE INTERSECTION. AIRCRAFT 2 ADVISED LC THAT THEY WERE REJECTING THEIR TAKEOFF AND EXITING THE RUNWAY AT TWY L. THE CLOSEST PROXIMITY FROM THE ASDE ANALYSIS WAS 176 FEET LATERAL.
